    Social Services Coordinator Barcelon Associates Mgmt. Corp. - Pleasanton, CA $24 - $27 an hour We are seeking a Social Service Coordinator to serve the seniors at a 200-unit apartment community in Pleasanton, serving low-income elderly residents. In order to serve these seniors well, you must have patience, be a good listener, and be able to work with them one-on-one, in groups, and sometimes with their families. This position provides an incredible opportunity, for the right person, to join a dynamic and dedicated staff serving wonderful and deserving seniors who live on their own. This is a full-time, 30-hours/week, Monday Friday, non-exempt position. ESSENTIAL FUNCTIONS & DUTIES * Provides general case management and referral to services to all residents, and may perform formal case Management, if such is not available in the community. * Initiates social service assessments on all residents within 30 days of move-in, and annually. * Establishes linkages with all agencies and service providers in the community. * Maintains a directory of providers for use by both community staff and residents. * Refers and links residents to service providers such as case management, personal assistance, meals-on- wheels. * May develop case plans in collaboration with assessment services in the community. * Monitors the ongoing provision of services from community agencies and keeps the case management and provider agencies current with the progress of the resident. * May set up volunteer support programs with service organizations in the community. * Assists residents to build support networks for themselves.

    • May provide training to residents in the obligations of tenancy. * May educate other staff on the management team on issues related to aging-in-place and service coordination, assisting them to better serve residents. * Encourages residents to be proactive in meeting their social, psychological, and physical needs. * Identifies barriers to providing services and problem solves to overcome these barriers. * Advocates and negotiates on residents behalf for adequate, timely and cost-effective provision of aging services. * Pursues avenues for additional services through private, local, state, and federal sources.
    • Creates care service plans for all residents receiving service coordinator assistance. * Works with hoarders and other unique individuals to promote meeting minimum residency standards. * Assists residents (and their families) in determining their need for a higher level of care, and assists with placement and transition. QUALIFICATIONS
    • A BACHELOR OF Social Work or degree in Gerontology, Psychology, or Counseling is preferable; a college degree is fully acceptable. However, individuals without a degree, but with appropriate work experience may be hired. * Training in the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal liability issues, drug and alcohol use and abuse among elderly, mental health issues, etc. * 2-3 years experience in social service delivery with senior citizens. * Demonstrated working knowledge of supportive services and other resources for senior citizens. * Demonstrated ability to advocate, organize, problem-solve and provide results for the senior persons served. OTHER REQUIREMENTS * Must have own vehicle and be able to drive to hospitals, meetings, etc.
    • Able to get to residents apartments regularly.
