    Hospice F/T RN Admit Nurse- Bi-Lingual - Join Our Compassionate Team at Optimal! Department: Nursing Location: Modesto, CA JOB SUMMARY: The Admissions Nurse is responsible for the admission of patients/families onto service with Optimal Hospice. The admission needs to be done in a timely manner upon patient/family and referral source request. The Admissions Nurse conducts the intake evaluation, and completes the admission process in a manner that promotes an interdisciplinary team approach to care. This approach respects patients/families, meets Medicare/Medicaid conditions of participation and accreditation requirements, and reflects the mission and philosophy of Optimal Hospice. The Admission Nurse is responsible for educating patients and their families, along with promoting the concept of hospice and its benefits to physicians, discharge planners, case managers and other internal and external healthcare professionals. QUALIFICATIONS: 1. Is currently licensed as an RN through the California State Board of Registered Nurses. 2. Must have a minimum of (a) one year of experience as a professional nurse within the last three years; or (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license. 3. Complies with accepted professional standards and principles. 4. Understands Hospice philosophy, principles of death/dying. 5. Has presented a pre-employment physician?s health clearance including a negative TB skin test and/or CXR and other tests as required by the organization?s policy. 6. Must be a licensed driver with an automobile that is insured. 7. Possesses and maintains current CPR certification. 8. Has satisfactory references from nursing school, previous (or current) employers and/or professional peers. 9. Has excellent observation, good nursing judgment and communication skills. 10. Ability to provide good written documentation in a timely manner. 11. Is self-directed with the ability to work with little supervision. 12. Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team. JOB DUTIES: 1. Possesses understanding of the Hospice program and benefits. 2. Obtains orders for and procures medications and needed DME/supplies. 3. Performs physical assessment of patients? condition, safety, knowledge, pain and symptoms. 4. Initiates and develops the Plan of Care. 5. Educates on disease understanding, progression, and immediate interventions. 6. Documents assessment, current medications and initiates appropriate care plans. 7. Responsible for assessing patient?s medical status, planning, teaching and implementing immediate medical interventions on day of admission. 8. Actively participates in coordination of all aspects of patient?s care, in accordance with current professional standards and practice. 9. Participates in ongoing interdisciplinary comprehensive assessment. 10. Contributes to patient and family counseling and education. 11. Collaborates with attending physician, Hospice Medical Director, Clinical Coordinator and Social Worker and Spiritual Chaplain regarding patient admission and follow up needs. 12. Responsible for completion of all elements of admission process 13. Communicates admissions information, issues and concerns in patient care meetings to assure smooth transition and continuity of care with other assigned staff, including a detailed report to the assigned Case Manager and assigned Social Worker 14. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 15. Fulfills the obligation of requested and/or accepted case assignments. 16. Actively participates in quality management program. 17. Do other duties as assigned, including work in other areas to cover absences of relief to equalize peak work periods or otherwise meet the needs of the organization.
