Brookdale Healthcare Services is looking for a Home Health Scheduler to provide services in the Destin area. Join a dynamic team of health care providers focused on health, wellness and prevention for all our patients. Your responsibilities: * Coordinate the scheduling of clinical visits for care team in compliance with physician approved orders and clinician plotted visits approved by Clinical Services Manager * Assist clinicians with assuring all visits are properly assigned and scheduled on Electronic Medical Record (EMR) devices * Responsible for monitoring and scheduling home health therapy reassessment visits to meet home health reassessment requirements * Prepare reports for scheduling tracking and participates in agency meetings to ensure patient and associate scheduling needs are met Required skills and qualifications: * High school diploma or General Education Degree (GED) required * Home Health Scheduling experience preferred * One year of experience scheduling and working with older adults or in a related healthcare setting * Must have knowledge and working capability of PCs, including Microsoft Word and Excel If you're a Brookdale associate, please consider referring someone through the Good People Program!
