    Job Description

    The GROUP FACILITATOR is responsible for facilitating group therapy at the outpatient level of care (PHP & IOP).  The Group Facilitator participates in de-escalation or crisis intervention when necessary. The facilitator communicates pertinent information to the primary treatment team as needed.

    ESSENTIAL JOB FUNCTIONS:

    • Collaborates with Program Director and/or Clinical Supervisor and other professional staff to help develop comprehensive program of therapy, evaluation and treatment.
    •  Facilitates group therapy, workshops, and educational groups as directed by the program schedule.
    • Responsible to Complete all Group notes immediately after each group you lead.
    • Document all services provided in clients medical record.
    • Knowledge of company’s policy and procedures, and Joint Commission standards and regulations. Adhere to all regulations and documentation requirements in regards to client care and compliance of all client care. Ensure that clients charts are always up to date.
    • Attends staff meeting, and periodically providing training regarding therapeutic services and, mental health topics as needed
    • Assesses suicidality and possibility for danger to self or others.
    • Charts each resident’s progress throughout the treatment experience.
    • Charts after each individual, group, and family session.
    • Educates each resident as needed.
    • Begins and ends groups on time.
    • May carry a small caseload as needed, which will include:
      • Completes the Biopsychosocial assessment within one day of admission (parent and/or resident components**)
      • Starts Comprehensive Treatment Plan on day of admission and completes ]Treatment Plan and Integrated Treatment Summary within three days of admission, with all necessary signatures obtained.
      • Identifies weekly treatment goals and writes contracts relative to individual client needs
      • Writes and facilitates the Discharge Summary by the day of discharge, and assures that follow up appointments are scheduled within seven days of discharge.
      • Individual psychotherapy sessions, 3 times weekly for each client
      • Facilitates family psychotherapy sessions at least once each week.Creates a written plan for continuing care (DC plan) and discharge summary by day of discharge.
      • The Primary Therapist attends all treatment team meetings, all treatment review and goals setting groups, in-service training’s, and general staff meetings and clinical retreats.
      • The Primary Therapist “oversees” the daily treatment experience of each of his/her resident clients.
      • The Primary Therapist may provide additional clinical support for other members of the clinical team in their absence.
    • May support with Utilization Review as needed, which will include:
      •  Interfaces with insurance companies, providing appropriate clinical information to obtain initial certification and continuing stay certification as required.
      • May be required to travel to other facilities to attend trainings.

      

    This is a full time position at one of our outpatient programs. 

  • Qualifications

    Qualifications

    • Group facilitator must be licensed or working towards licensure (LMFT, LPC, LCSW, LMHC, PsyD) in the state in which they are applying
    • Center for Discovery utilizes Associate Therapists (MFT, Social Workers, and Professional Counselors) and Psychological Assistants under the supervision of an independently licensed mental health provider
    • Must maintain license and certification required by state law in good standing
    • Must understand the therapeutic process, understand confidentiality laws, and have strong knowledge of professional ethics
    • Successful candidate must submit to post offer, physical examination/medical history check, and TB test. Must have valid CA. driver’s license and liability insurance if driving personal vehicle on our property
