In collaboration with the treatment team, the Coach develops a plan of care and provides in the moment interventions to be used in the clients natural environment in order to increase positive behaviors related to coping, anger, impulse, social and self-control skills. The Coach conducts daily documentation and charting and works closely with supervisors to coordinate services for the client. JOB RESPONSIBILITIES: ADMINISTRATIVE Assists with planning and implementing a comprehensive TBS program for clients and caregivers. Reviews facility policies and procedures as part of the facilitys interdisciplinary team to assure compliance with federal and state regulations. Participates in Utilization Review process as requested by the TBS Manager. Understands and meets all government requirements for Therapeutic Behavioral Services documentation. Coordinates working schedule and all daily client concerns with Team Leader. Assures timely entries in the clients charts to include, but not limited to: a care plan to address strengths, problems, needs, and interventions, substantiation of interventions, progress toward, and/or completion of goals, and discharge summaries. Consults with TBS Clinical Supervisor and other departments regarding interdisciplinary issues. Works with mental health practitioner of clients to promote congruence between theraoy and TBS goals/interventions and prevent duplication of service. Ensures that department billing and productivity standards are met. Serves as an active contributor in Treatment Team and other client planning meetings at request of TBS Manager. 11. Ensures the delivery of services to the client while accounting for the employees safety as well as the well-being of the client. ADVOCACY Works with the interdisciplinary team to promote and protect client rights. Prevents and addresses child abuse as mandated by law and professional licensure. Works with clients and caregivers to take a more proactive role in self advocacy to improve the resources available to the family. Responds to issues identified by clients and families to determine satisfaction with services. BEHAVIORAL Coordinates and provides services to adolescents and children under the supervision of the TBS Clinical Supervisor, Team Leader and Manager. Services will frequently involve one to one interaction with a specific client for extended periods. Participates in the development of a thoroughly complete interdisciplinary plan of care for each client that identifies the behavioral needs/issues of the client, the goals to be accomplished for those needs/issues, and the appropriate TBS interventions to promote skills-building. Delivers therapeutic interventions to assist clients in managing their behavior in order to maintain their placement, transition from a higher lever of care or to avoid acute psychiatric hospitalization. 4. Provides education to caregivers and family members to help them learn skills-building interventions which can be used with client in home setting during and after the treatment process. 5. Participates as part of the interdisciplinary care team in providing skills-building interventions to resolve behavior or mood problems. 6. Works in tandem with the clients mental health provider to assure continuity of care. 7. Participates in treatment team meetings as indicated. DISCHARGE PLANNING Ensures that client discharge goals are identified at admission. 2. Works with caregivers and family members to identify strengths and needs to ensure an appropriate discharge plan is formulated. 3. Responsible for communicating to treatment team the estimated discharge date. Makes referrals as needed for post discharge care to appropriate agencies. EDUCATION/SUPERVISION Participates in new employee orientation, including but not limited to educating staff regarding behavioral interventions and the TBS treatment process. Educates clients and caregivers regarding their rights and responsibilities, effective communication during the treatment process. Attends and participates in facility and community education and professional development programs. Participates in individual and group supervision activities. Attends department and facility staff meetings. Other duties as assigned. GENERAL Maintains 65% billable service delivery (26/wk) To work with mental health practitioner of clients to promote congruence between therapy and TBS goals/interventions and prevent duplication of service Other duties as assigned TOTAL 100% JOB QUALIFICATION STANDARDS Experience One year direct service experience treating emotionally disturbed children and transitional-aged youth. Education, Professional Training, Technical Training or Certification Bachelors degree in Psychology, Sociology, or Human Services field required Must hold a valid drivers license and ability to travel, and valid car insurance. Knowledge/Skills This position requires that the employee is able to read, write, speak and understand the spoken English language to ensure the safety and wellbeing of our patients and visitors at the work site when responding to their medical and physical needs. Bilingual Spanish speaker preferred Maintains confidentiality in matters of agency operations, personnel, and clients Maintains positive communication with all agency staff, clients and visitors Exercises good judgement in performance of duties and responsibilities Supports existing agency policies, principles and mission. Competent in values, ethics, and theoretical perspectives; assessment; intervention; aging services, programs, and policies. Knowledge of biological, psychological, social, emotional, spiritual, practical informational and financial needs and challenges experienced by the patient/resident, family, and caregiver.
