External Client Care Coordinator | Full-Time

Service Areas: Pottsville, Schuylkill Haven, Saint Clair, Minersville, Ashland, Girardville, Frackville, New Philadelphia, New Ringgold, Tamaqua, and surrounding communities.

Build relationships that make a difference. Comfort Keepers is looking for an organized, people-focused External Client Care Coordinator to help connect families with quality in-home care while supporting an outstanding client experience.

What You'll Do

Build strong relationships with clients, families, caregivers, and referral partners

Conduct in-home consultations to understand client needs and recommend services

Coordinate care plans and caregiver schedules to ensure quality service

Follow up with clients to maintain satisfaction and address concerns

Collaborate with office staff to provide seamless care coordination

Maintain accurate client records and required documentation

Develop partnerships with healthcare providers and community organizations

Support local networking events and community outreach initiatives

Help identify opportunities to improve the client experience

What We're Looking For

Excellent communication and relationship-building skills

Strong organizational and time-management abilities

Ability to prioritize multiple tasks in a fast-paced environment

Professional, compassionate, and customer-focused mindset

Comfortable using computer systems and office software

Experience in healthcare, client services, scheduling, or coordination is a plus
