    PRIMARY RESPONSIBILITIES:

    • Acts as a resource for physicians and staff regarding operations, network provider options, and systems
    • Outreach to enrollees to provide assistance or guidance, such as information about out-of-pocket costs, identification of service providers, encouragement for and facilitation of preventive screenings and chronic care services
    • Triage to services, such as Care Management and other internal services. Works collaboratively with the Network Management, and physician offices to ensure optimal patient experience and clinical outcomes
    • Assists in the distribution of materials to the provider's offices.
    • Assists in the enhancement of communication and care coordination between multiple physicians caring for an patient

  • Qualifications

    REQUIRED QUALIFICATIONS:

    • HS Diploma or GED
    • 2+ years of physician office practice operations or patient customer service case management
    • Working knowledge of medical terminology and / or equivalent combination of education and experience
    • Proficient with MS Word, Excel and Outlook

    PREFERRED QUALIFICATIONS:

    • Certified Medical Assistant
