    Community Based Coordinators (CBCs) will be in frequent communication in Texas with providers in -person, mail, phone and fax with program changes and to answer questions. CBCs will assist contracted providers with analyzing member care, trending quality compliance at the provider level, and developing action plans and community programs (per market need) to support provider practices in continuous quality improvement using approved clinical practice guidelines, HEDIS, CMS, NCQA, ADA, DQA and other tools. CBCs may conduct provider fluoride training and provide educational materials related to Texas Health Steps (THSteps), preventive visits, sealants and other evidence-based interventions proven to improve outcomes in Medicaid or other high-risk populations.

    From a community standpoint, CBCs plan, coordinate, and implement member engagement and educational programs in the community (per market need). They carefully monitor, then, provide consultation and direction to ensure programs and services are implemented at the highest standards and beneficiaries receive the highest level of care. The CBCs will work closely with community organizations, members and providers to identify barriers to care and alleviate barriers in a manner reflecting local values and resources. The CBCs will seek to partner with local community health departments or non-profit federally qualified health centers (FQHCs) who are already a part of our network to connect with local school districts to offer school-based dental programs. They will work diligently to develop, implement and monitor these programs to ensure that our members benefit from state-of-the-art prevention programs that respect local guidelines.

    CBCs will play a key role in cooperation and coordination with THSteps outreach to ensure prompt delivery of services to members who miss dental checkups. On a monthly basis, UnitedHealthcare Dental will retrieve data to target outreach to them. CBCs will also work closely with HHSC, outreach programs, and THSteps regional program staff and agents to ensure prompt delivery of services to Farmworker Children and other migrant populations, and to coordinate with Head Start and other migrant farm worker organizations.

    PRIMARY RESPONSIBILITIES:

    • Support effective deployment of a quality improvement program at the practice level through strategic partnerships with participating dentists and practice staff while assessing trends in quality measures and identifying opportunities for quality improvement
    • Participate, coordinate, and/or represents the Dental Plan at community based organization events, clinic days, State health department meetings, and other outreach events focused on quality improvement, member dental education, and disparity programs as assigned.
    • Report individual member quality of care concerns or trends of concern to the Dental Plan A&G representative as needed
    • Provide education to Providers and office staff on state-mandated quality metrics specifications as well as HEDIS and DQA measures
    • Document and refer providers' non-clinical/service issues to the appropriate internal parties, to include Provider Relations and the Plan Dental Officer

  • Qualifications

    REQUIRED QUALIFICATIONS:

    • Associate's Degree
    • Intermediate level of proficiency with PC based software programs and automated database management systems required (Excel, Word, PowerPoint)

    PREFERRED QUALIFICATIONS:

    • Bachelor's Degree
    • Licensed Dental Hygienist
    • 5+ years of state contract based or healthcare experience
    • Microsoft Access experience
    • Experience with Medicaid
    • Solid understanding of ADA codes and general dental terminology

    SOFT SKILLS:

    • Excellent communication, time/project management, problem solving, organizational and analytical skills
    • Ability to identify solutions to non-standard requests and problems
    • Ability to meet the qualifications listed in this posting
