    Summary

    Peer Support Specialist responsible for coordinating care for individuals engaged with the correctional health system. Maintains relationships with correctional health facilities to coordinate referral to treatment from correctional health facility to MAT provider. Conducts weekly in-reach, including educational sessions, groups, and peer support to individuals residing in and affiliated with the criminal justice system. Works closely with assigned Clinic Managers, Operations Manager, Regional Director, and Chief Medical Officer to assist with appropriate coordination with correctional health. Maintains accurate documentation and tracking for assigned clients. The work involves working collaboratively with staff of all levels to help facilitate client recovery. Position will promote pro-social community activities, recovery, self-advocacy, self-help, and development of a support system. The work will be performed in a wide range of settings, including any local CMS clinic location, community-based outpatient settings, community agencies, correctional health facilities, and in transport vehicles.

    Primary Job Responsibilities

    Correctional Health Referral Coordinator works with clients receiving services from Correctional Health and Criminal Justice Systems.

    Weekly in-reach including groups, peer support, and educational sessions at the correctional health facility.

    Provides documentation and reporting of client encounters.

    Maintains appropriate coordination of care with all systems and provides documentation as needed/ requested from multiple system partners.

    Substance abuse rehabilitation services and education of assigned caseload. Provides coordination of care between mutual providers and act as a liaison between clients and outside agencies and programs. Provides appropriate treatment and service referrals as needed. Uses past experiences to provide individualized, ongoing guidance, coaching, and support to individuals seeking or in active recovery.

    Assists clients in identifying effective coping techniques, self-help strategies, and increasing social support networks.

    Observes all rules of confidentiality relating to clinical information and treatment, both internally and externally. Adheres to deliverable timelines, both internally and externally.

    Works in a variety of settings (jails, drug courts, etc.) providing support, guidance, and education for individuals seeking or in active recovery.

    Maintains constant professional standards by observing guidelines established within the CMS Code of Ethics. Facilitates or assists in group settings to promote harm reduction and recovery.

    Attends required meetings: Clinical staff meetings, clinical supervision, workshops, seminars and training, etc. as assigned by the Regional Director, Director of Quality Management, Human Resources, Clinic Manager, Clinical Coordinator and/or the Chief Medical Officer. May provide in-service trainings to staff.

    Community Based Services of travel time, waiting for system partners, and non-billable services during community-based practices. Providing program information and assessment information to non-clients.

    QUALIFICATIONS

    Education, Certification and Experience Requirements

    Minimum one year in recovery

    Experience with navigating a behavioral health system and other community support programs is preferred

    Valid driver's license is required; required clean MVD record for 36 months

    A sixteen (16) hour online e-based academy administered by the department

    Proof of a minimum of forty hours of peer service delivery training of three equivalent years formal experience in peer service delivery as a peer navigator, peer supporter, peer specialist, or peer recovery coach

    o Passing the department [LW 2]peer recovery supporter exam

    o Submission of a signed certified peer recovery supporter code of ethics established by the department

    o Submission of a completed application[LW 3]

    o Submission with the completed application, the results of a bureau of criminal identification and investigation criminal records check conducted within thirty days prior to submission

    Ability to pass background check and become badged to enter correctional heath facilities is required.

    Experience in substance use disorder treatment and methadone treatment is desirable.

    Basic computer knowledge is necessary.

    (2) year(s) of experience in opioid treatment program preferred

    year experience in corrections or criminal justice related position

    Tools and Equipment Requirements

    The ability to use a phone, computer, printer, and copier is required.

    Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint.

    The ability to use the internet and various web browser software is required.
