    CORRECTIONAL HEALTH LIAISON JOB SUMMARY

    The Correctional Health Liaison for Community Medical Services is responsible for completing intakes, facilitating group counseling, individual counseling for and maintaining a caseload of justice-involved individuals. Maintains accurate documentation for assigned clients and provides substance abuse rehabilitation, support, and education services to assigned patients. Responsible for interfacing with community justice systems in assigned geographic areas, including attendance at drug court staffings, performing jail in-reach and ensuring day-to-day operations of justice-related grants are carried out. Works to identify integration and collaboration opportunities within the justice systems. Supports development and implementation of programs and works with other departments on program development activities as they relate to justice integrations, as assigned.ESSENTIAL DUTIES AND RESPONSIBILITIES (Includes the following; other duties may be assigned)

    Interfaces with clients receiving services from the correctional health system and Community Medical Services. Coordinates all client entrances, transfers and discharges between CMS and all correctional health settings. Provides coordination of care, follow up, direction and assistance for all stages of treatment for clients involved in the correctional health system.

    Implements day-to-day operations of justice-related grants, including the development of deliverables, as required in grant narratives.

    Facilitates group therapy sessions, which may include Intensive Outpatient Moral Reconation Therapy or Cognitive Behavioral Therapies.

    Performs jail in-reach to identify individuals in need of opioid use disorders and assists in coordinating care to ensure individuals identified receive necessary services and support.

    Attends weekly Drug Court staffings, coordinating care for identified clients, including coordination of care with external agencies as necessary to meet client needs.

    Maintains appropriate coordination of care with all systems and provides documentation as needed/ requested from multiple system partners.

    Maintains Community Relations documentation/ log for all contracts, meetings, conversations, presentations and other outreach activities. Maintains appropriate documentation of coordination of care efforts.

    Establishes and attends local monthly Recovery Advisory Board meetings, meeting grant and other regulatory requirements.

    Attends staffing or Adult Recovery Team meetings as needed.

    Supports program development and implementation. Works with project and program management team.

    Attends required meetings: Clinical staff meetings, clinical supervision, workshops, seminars and training, etc., as assigned by leadership. May provide in-service trainings to staff.

    Other duties as assigned.

    QUALIFICATIONS

    Bachelor's Degree or Behavioral Health Technician as defined by ADHS

    Experience in substance abuse rehabilitation and methadone treatment is desirable.

    Bilingual is preferred

    1 year experience in corrections or criminal justice related position

    Intermediate computer knowledge is necessary and capability of keeping accurate client files and reports

    Effective follow up and training capabilities in order to address required changes or areas for improvement
