    POSITION SUMMARY

    The Community Medical Services (CMS) Community Impact Manager reports to the Regional Director and is responsible for developing new community relationships and programs. The Community Impact Manager develops and maintains community relations with community partners, which includes but is not limited to, community coalitions, health homes, sister agencies, specialty providers, local hospitals, clinics, law enforcement, fire and EMT services. This role creates and maintains documentation related to outreach and collaborations via CRM. The Community Impact Manager oversees and implements communications with local agencies and maintains accurate documentation of center of influence interactions within the community. This role is responsible for managing projects and new community initiatives from start to completion. The Community Impact Manager collaborates with community partners to gain an understanding of their needs and helps to create the most appropriate plans and projects to serve the community and CMS partners. This role works with CMS clinic staff to establish best practices when receiving referrals from outreached agencies or community partners.

    PRIMARY JOB RESPONSIBILITES

    Maintains documentation/logs in the CRM platform for all outreach activities including but not limited to contracts, meetings, and conversations. Maintains appropriate documentation of coordination of care efforts.

    Establishes rapport with outside agencies, to include but not limited to hospital systems, correctional health systems, EMS, police departments, etc.

    Attends meetings and participates in activities for improvement of coordination of care for OTP clients. Responsible for building and being the integral contact for coordination of care between OTP clients and community agencies.

    Provides documentation of all possible client/agency encounters.

    Maintains appropriate coordination of care with all systems and provides documentation as needed/ requested from multiple system partners.

    Attends required meetings including clinical staff meetings, clinical supervision, workshops, seminars, trainings, and other meetings as assigned by the Regional Director.

    Maintains project management related documentation for new community programs including but not limited to timelines, tasks, workflows, minutes, action items, and decisions. Effectively provides project status updates.

    Work closely with internal and external teams to obtain in depth understanding of the documentation, process, and billing requirements for new community programs

    Responsible for collecting, maintaining, and analyzing KPI for direct reports as well as tracking client referrals and established in progress community partnerships

    Community Based Services that include providing program information and assessment information to non- clients. Providing clinical support, including assessments, to special populations and projects.

    Attends provider and community meetings for purpose of advocating, educating, and supporting system partner relationships.

    Resolves or escalates issues in a timely fashion

    Performs other duties as assigned

    QUALIFICATIONS:

    Education, Certification and Experience Requirements

    Bachelor’s or Associate’s degree in related field, or related experience

    Experience in an Opioid Treatment Program setting is preferred

    Experience with various community partners and government agencies (i.e. other behavioral health agencies, RBHAs, Dept of Child Services, correctional health/criminal justice, and others) preferred

    Basic computer skills Extensive knowledge of Microsoft Office applications

    Ability to be self-directed in the completion of individual work assignments

    Detailed, organized, and has efficient record keeping skill

    Tools and Equipment Requirements

    The ability to use a phone, computer, printer, and copier is required.

    Frequent use of Microsoft office products, including but not limited to Outlook, Word, Excel, and PowerPoint.

    The ability to use the internet and various web browser software is required.

    Physical Working Conditions and Office Setting Description

    Requires sitting and standing associated with a normal office environment.

    Manual dexterity using a calculator and computer keyboard.

    Requires prolonged sitting, standing, frequent bending, stooping, or stretching.

    Some lifting may be required.

    Frequent and prolonged typing and frequent and prolonged operation of computer, keyboard, and telephones required.

    Requires occasional use of fax machines, telephones, copiers, and other office equipment.

    Employee must regularly lift and/or move up to 10 pounds, frequently lift and/or move up to 25 pounds and occasionally lift and or move up to 50 pounds.
