JOB DESCRIPTION Job Title: Youth Services Family Coordinator Department: Recovery Services Reports to: Clinical Supervisor Supervises: None FLSA Status: Non-Exempt Pay Grade: C3 Job Type: Regular, Part-Time ---------------------------------------------------------------------------------------- General Functions: The Family Coordinator promotes coordination and integration of family-centered care to ensure participation and involvement of the entire family, and provides individualized case management services as needed to the parents, caregivers, and other treatment-involved family members of adolescent youth experiencing substance use disorders and/or co-occurring mental health and substance use disorders. The position will support individualized, integrated mental health and recovery treatment plans, and will link all participants with needed community-based services, including pathways to permanent housing, and housing assistance. The Family Coordinator will work to establish an effective documentation platform for all services, and will work closely with the community partners and other providers in the relevant system of care. Duties and Responsibilities : Work directly with the families/caregivers of adolescent youth participants, program partners, and other local social service and health care providers to engage families and address their relevant needs on an individual basis. Provide limited outreach services to families of adolescents experiencing SUD/COD, and case management as needed for an average service period of three months. Participate in regular CITC and community partner case management meetings. Documents all activities in participant's electronic health record within the Recovery Services Case Management System (RSCMS). Utilize Motivational Interviewing (MI) techniques while engaging participants and assisting with case management. Utilize Trauma Informed approach while engaging participants. Utilize a Positive Youth Development framework while engaging participants. Attend staff meetings, trainings, coalition meetings, and other community meetings as requested to advocate for and seek expansions of the youth-serving system of care. Ensure connections are made with community providers pre and post discharge, with a primary focus on connections to family-centered care supporting long term wellbeing. Attends RS staff meetings and works collaboratively with other staff members during participant case staffing regarding transitional and discharge planning needs, to facilitate a successful transitional from treatment services to the community. Participate in training in areas related to improvement of service delivery. Maintain a respectful relationship with participants, former participants, and family members. Use culturally responsive practices with participants. Maintain a positive working relationship with the internal and external continuum of care. Maintain confidentiality according to regulations, policies, and procedures. Perform all related duties as assigned . Job Specifications: Excellent verbal and written communication skills. Proficient with PCs and standard office equipment. Demonstrated ability to make decisions and exercise good judgment. Demonstrated ability to work independently with minimal supervision. Demonstrated ability to work under pressure and prioritize workload. Strong public speaking and organizational skills. Strong problem solving skills. Demonstrated ability to coordinate multiple activities. Excellent time management skills. Must have strong writing skills with attention to detail and accuracy of fact reporting. Demonstrated ability to work with individuals from diverse backgrounds. Minimum Qualifications: Associate's degree in Human Services or related field, or an equivalent combination of education and relevant experience may be substituted. Experience providing case management services. Valid Alaska Driver's License and be insurable under CITC's automotive insurance, which requires a driver to be at least 21 years of age. Continued employment is contingent upon receipt of a satisfactory state and federal background check. One year experience working with family-centered interventions, and/or youth experiencing substance use disorders and SUD/COD. Preferred Qualifications: Bachelor's degree or higher in Behavioral Sciences (Social Work, Human Services, Psychology or related field) One year experience working with reentry populatio n.
