    What You’ll Do:

    Oversee the delivery of high-quality nursing care, including client assessments, medication administration, wound care, and client/family education.

    Develop, implement, and evaluate individualized care plans tailored to each client’s health needs.

    Monitor vital signs and ensure treatments are delivered accurately and consistently.

    Supervise, mentor, and train nursing staff and caregivers, ensuring compliance with New Jersey regulations and agency policies.

    Conduct new client assessments and ongoing quality assurance visits.

    Lead and manage CHAP accreditation readiness, including implementation, documentation, staff education, and ongoing compliance.

    Prepare for and participate in CHAP surveys and follow-up activities.

    Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to support person-centered care.

    Maintain accurate, timely, and compliant clinical documentation.

    Organize and lead training sessions focused on home care best practices and regulatory standards.

    Support recruitment, evaluation, and retention of skilled nursing staff and caregivers aligned with organizational values.

    Address client and family concerns with empathy, professionalism, and effective care coordination.

    Assist with relationship-building and outreach to referral sources and healthcare partners.

    Ensure compliance with New Jersey state and federal regulations governing home health services.

    Perform other duties as assigned.

    Are You the Right Fit?

    Active RN license in the state of New Jersey.

    Minimum of 3 years of nursing experience, including home care experience and demonstrated leadership responsibilities.

    Required: Demonstrated knowledge of CHAP accreditation standards, including agency readiness, implementation, ongoing compliance, documentation, and participation in CHAP surveys.

    Strong clinical assessment skills; home care and geriatric experience preferred.

    Proven leadership, communication, and interpersonal skills.

    Ability to work independently, manage multiple priorities, and adapt to evolving needs.

    Proficiency with electronic health records (EHR) and healthcare technology.

    CPR and First Aid certification required; CPR Instructor certification is a plus.

    Strong knowledge of New Jersey home health regulations.

    Reliable transportation and willingness to travel locally within New Jersey.
