In this role you will be responsible for: - Provides medical social work service including identification, assessment, interventions, planning, counseling, discharge planning and community resource services for medical/surgical patient - Facilitates the utilization review process along with the Case Manager - Coordinates placements to long term care and specialty hospitals - The MSW (along with the Maternity Social Worker) is the main resource for all education and intervention related to domestic violence - Every other weekend required If you possess the following qualifications, please apply immediately: - Bachelors degree required - Licensure and/or Certification as required by the Commonwealth of Pennsylvania - 2+ years' social work experience in a health care setting or social service agency required - CCM eligible required Physical Demands: - Physical Requirements: Sitting, standing, driving, lifting, carrying, pushing/pulling, walking, agility (bending, stooping, twisting, crouching, kneeling, turning), dexterity (handling, fingering, reaching)
