Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community such as 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults as well as outpatient services such as medication management, case management, and counseling.

Job Summary

The FACT-IL / MFT Support Nurse (LPN/RN) provides treatment, rehabilitation, clinical case management, care coordination, and support services to individuals served by the FACT-IL program. Additionally, the nurse completes nursing assessments within scope of licensure, assesses physical health needs, coordinates referrals to community health care providers, and ensures safe and effective medication coordination, including injectable medications, in conjunction with the individual's Psychiatrist or Psychiatric APRN. The nurse serves as primary case manager for an assigned group of individuals and promotes recovery, empowerment, self-determination, and shared decision-making for each FACT-IL participant. FACT-IL provides community-based behavioral health treatment and support to adults with serious mental illness using a multidisciplinary clinical case management model with medical services, wellness management, and recovery support. Services are designed to decrease crisis episodes, promote independent functioning, improve community tenure and involvement, and support successful transitions to community-based care.

The nurse also provides limited nursing support to the Forensic Multidisciplinary Team (MFT), focused on coordination with the FACT-IL/MFT Combined Psychiatric APRN, pharmacy, and medication room operations, and consultation to MFT staff on nursing and medication room processes.

Responsibilities

Provides treatment, rehabilitation, and support services in accordance with FACT-IL service expectations.

Provides and oversees nursing and medical services for individuals served by the FACT-IL program.

MFT Medication Support and Coordination.

Assures the accuracy, completeness, and confidentiality of clinical records.

Maintains administrative records and responsibilities.

Maintains communication with supervisor relative to program functioning, unusual events, problems, as well as successful achievement.

Assists in the training and development of new employees.

Attends trainings, in-service sessions and staff meetings.

Qualifications

Graduation from an accredited school of Practical Nursing required. Associates or Bachelors degree in nursing preferred.

Six (6) months of proven experience as a RN, LPN or similar role. Experience working with individuals with mental health related issues preferred.

Valid Florida license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) required.

Each member of the FACT team must meet MHRC/RBHS vehicle driver requirements, including a valid Florida Driver's License, automobile insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits, and no more than eight points on their license for any combination of violations.

Position Details Full Time Shift: Monday through Friday (rotating weekends, on-call, and holidays)
