Company Overview
Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community. Services include 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults, as well as outpatient services such as medication management, case management, and counseling.
Job Summary
The FACT Team Leader administers and directs Florida Assertive Community Treatment (FACT) Program, a clinical team, which assumes responsibility for directly providing needed treatment, rehabilitation, and support services to persons with serious mental illness. Promotes recovery, empowerment, self-determination and decision-making for each FACT participant.
The FACT (Forensic Assertive Community Treatment) Program is a multidisciplinary clinical team that provides comprehensive, community-based services to adults with severe and persistent mental illness. The program is designed to reduce hospitalization, increase community tenure, and enhance independence, autonomy, and quality of life.
FACT team members provide a wide range of services, including advocacy, crisis intervention, assessments, treatment planning, case management, medication support, and coordination of care. Additional services include community referrals, symptom education, vocational and educational support, social skills development, and assistance for families and caregivers.
Most services are provided in the community, with approximately 15% of time spent in the office. Company vehicles are available; however, team members must also use their personal vehicles as needed and are eligible for mileage reimbursement.
The FACT team operates during and outside of standard business hours, including rotating weekend, holiday, and on-call responsibilities.
Responsibilities
Clinical
Assesses and screens individuals for FACT services and coordinates admissions with the psychiatrist.
Assigns staff to Individual Treatment Teams (ITT) and designates primary case managers.
Oversees development of initial and comprehensive assessments and recovery plans.
Provides clinical supervision and leads treatment planning and recovery meetings.
Delivers direct clinical, rehabilitation, and support services as an active team clinician.
Coordinates daily team meetings to prioritize services and ensure timely care.
Provides clinical supervision, crisis support, and on-call coverage (24/7 rotation).
Ensures staff are trained in evidence-based practices (e.g., MI, trauma-informed care, WRAP).
Monitors risk, trauma, abuse, and neglect and ensures appropriate reporting.
Reviews clinical documentation for accuracy, timeliness, and compliance.
Administrative
Prepares required program reports and monitors performance and outcomes.
Oversees service delivery, documentation, and compliance with State contract and PACT model.
Develops corrective action plans in collaboration with leadership as needed.
Ensures appropriate staffing coverage across all shifts, including on-call.
Assists with budget monitoring and program financial oversight.
Maintains policies, procedures, and quality improvement processes.
Ensures compliance with medical records standards and regulatory requirements.
Staff Monitoring
Provides regular supervision and conducts staff meetings and peer reviews.
Identifies training needs and coordinates staff development.
Supports recruitment, hiring, onboarding, evaluation, and retention.
Partners with leadership and HR on employee relations matters.
Documentation Tool (Eleos Health)
Reduces time spent on documentation, allowing more focus on client interaction and care
Transforms brief notes or session conversations into complete, clinically appropriate documentation
Supports accurate, compliant documentation while allowing staff to review and finalize notes
Provides helpful insights from sessions to support care planning and continuity
Accessible on desktop or mobile for flexibility across different work settings
Qualifications
Master’s degree in Human Services or a related field required.
Three years of experience working with adults with severe and persistent mental illness required.
One year of supervisory experience required.
Valid Florida License as LMHC, LMFT, or LCSW required.
Experience with DBT therapy techniques preferred.
Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.
Proficiency in Microsoft Office, Outlook and the use of the Internet.
Must meet MHRC/RBHS driver requirements, including a valid Florida driver’s license, required insurance coverage, and acceptable driving record.
Ability to work in varied settings including community, hospital, and residential environments.
Strong clinical judgment with ability to respond to emergencies and high-risk situations.
Strong leadership and communication skills with ability to collaborate across teams and systems.
Position Details
This position is a Full Time position (rotating weekends, on-call, and holidays).
