As a FIELD BASED COMMUNITY HEALTH WORKER (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.

    PRIMARY RESPONSIBILITIES:

    • Create a positive experience and relationship with the member
    • Keep the member out of the hospital by supporting regular visits to their primary physician
    • Keep member actively engaged with their primary physician
    • Support the member to ensure pick - up of their Rx
    • Proactively engage the member to manage their care
    • Provide member education
    • Support transitions of care
    • Help to keep members compliant with their care plans
    • Partner with care team (community, providers, internal staff)
    • Knowledge and continued learning of community cultures and values
    • May conduct HRA assessments if needed
  • Qualifications

    REQUIRED QUALIFICATIONS:

    • High School Diploma / GED (or higher)
    • Must have resided within the Queens area
    • Knowledge of culture and values of the community
    • Field based experience
    • Must currently reside within 30 miles of Queens, NY
    • Ability to travel up to 60% of the time within 30 miles or 1 hour from home base
    • Must have intermediate computer skills (be able to manipulate multiple programs)
    • United States Citizenship
    • Ability to obtain favorable adjudication following submission of Department of Defense eQuip Form SF86

    PREFERRED QUALIFICATIONS:

    • Bachelor's Degree (or higher) in Social Work and / or Health Care Administration
    • Licensed Practical Nurse (LPN), Certified Nursing Assistant / Home Health Aide, Medical Assistant
    • Bilingual in Spanish and English
    • Experience working in Managed Care
    • Knowledge of Medicaid population

    Please Note: In order to be considered for this position, you must be able to obtain an SF86 Position of Trust which is only available to U.S. Citizens. Our Facilities Security Officer will initiate this process post-offer acceptance. Failure to obtain this will result in termination from this role.
