POSITION DESCRIPTION

This non-traditional Social Work role blends the provision of individualized client care with the opportunity to improve the healthcare delivery system that supports the client. In this role, you will have the opportunity to work with individuals living in long-term care and through a therapeutic relationship; you will guide their transformation and give them the tremendous gift of self-sufficiency and self-confidence to successfully integrate in their supportive living environment. In addition to caring for the Geri-Psych population, you will be challenged by the needs of the comorbid mental and physical health disorders experienced by the Adult population now residing in long term care. As a leader, the care team will look to you to provide best practice techniques to engage and intervene in moments of patient crisis and stabilize the environment then, support the care team avoid the development of future crisis.

This Model of Care is unique to Optum and has a proven track record of both patient and organizational success. As integral member of the team, you will have the opportunity to work closely with both Behavioral Health and Medical Professionals to maximize the patient experience, while leveraging your talents where they are needed the most.

PRIMARY RESPONSIBILITIES

• Collaboration between medical and psychiatric APC to manage long term members chronic medical & psychiatric needs
• Facilitate/ Communicate with families about the progression of major neurocognitive disorders

REQUIRED QUALIFICATIONS

• Masters degree in Social Work
• Licensed Independent Clinical Social Worker, license active and unrestricted
• 2+ years of experience in a related mental health environment

PREFERRED QUALIFICATIONS

• Case management experience
• Experience in managed care
