    GENERAL SUMMARY:

    Under the supervision of the Nursing Supervisor or designee, the Registered Nurse provides skilled professional nursing care to the patient as prescribed by the physician and in accordance with the policies and procedures of Allied’s Home Health Division. 

    The Registered Nurse must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served by his/her department – adolescent through geriatric.  The Registered Nurse must demonstrate knowledge of the principles of growth and development over the life span; possess the ability to assess data reflective of the patient's status; interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs; and to provide care needed as described in the Home Health Division’s policies and procedures.

    EDUCATION:

     Graduate of an accredited Nursing Program required.

     WORK EXPERIENCE:

     Minimum of one (1) year of experience as a Registered Nurse preferred, preferably within the year prior to date of application.

    CHARACTERISTIC DUTIES & RESPONSIBILITIES OF JOB:

    Provides professional nursing care in the patient’s home utilizing all elements of the nursing process. Implements orders for medications and treatments as prescribed by the physician and all aspects of the nursing care plan. Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures in accordance with physician orders. Assesses patient conditions in every home health care visit.  Develops, implements and evaluates plans of care to ensures quality and safe delivery of home health care services. Monitors services delivered by home health care staff members. Participates in developing systems for evaluating home health care agency delivery of services.  Completes nursing assessment forms in a timely manner. Communicates patient assessment to appropriate individuals.

    Primary Responsibilities (including but not limited to the following):

     

    Program Management & Reporting: 

    • Participate in department meetings to discuss organizational objectives
    • Prepares status reports and presentations. Create and distribute ad-hoc reports
    • Maintain program project plan, schedules, and team collaboration tools
    • Monitoring vendor key performance indicators (KPIs) and service level agreements (SLAs)
    • Help establish and maintain vendor performance improvement plans
    • Ensure accurate vendor risk ratings are completed on a regular basis
    • Provide vendor data governance & information management support
    • Establish and maintain vendor management analytics & reporting dashboards
    • Research, collection, tracking and reporting of vendor service level agreements (SLAs)
    • Maintain information in the vendor management system
    • Track escalated issues and reporting of root cause analysis
    • Manage the archive and cataloging processes for all VMO documents
    • Track agreement renewal dates
    • Assist the team in the collection and analysis of vendor information
    • Perform invoice tracking against purchase orders as directed by the Vendor Manager
    • Develop and maintain processes, procedures, tools and templates, & vendor artifacts
    • Enhance and maintain vendor scorecards and maintain vendor surveys & reporting
