JOB OVERVIEW: I am committed to quality. It is at the core of everything I do and every decision I make. The Medical Social Worker is a qualified professional that provides skilled social services to patients on an intermittent basis in the home. My commitment will include:Helping patient and family with end-of-life care issues that encompass personal, financial, and physical decision making.Assisting with placement of patient when end-of-life care may be too cumbersome for the caregiver.Performing services in accordance with the plan of care and are coordinated with other team members.Assessing patients to identify the psychosocial, financial and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team report, call-in logs, and on-site evaluations.Participating in implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.Assuring the continuity of quality patient care delivered. Qualifications: Education: Masters degree in social work from a school of social work accredited by the Council of Social Work Education.Licensure: Current state license as a social worker. Must have valid drivers license.Experience: One (1) year social work experience in a healthcare setting.Skills: Requires excellent verbal and written communication skills with strong interpersonal skills.
