JOB OVERVIEW: I am committed to quality. It is at the core of everything I do and every decision I make. The Transitional Liaison acts as a consultant for patients and families to ensure all patients receive the appropriate information and counseling they may need to advance their care plan. My commitment will include:Receiving, interpreting and managing patient needs with the intent of assessing for appropriate care.Providing consultation on appropriate care services and/or specialty programs.Acting as a liaison with home health care clinical supervisors, physicians, and team members on behalf of patient/families when palliative care may be needed.Obtaining data on physical, psychological, social and other factors that may influence patient/family health status and sharing with clinical team. Qualifications: Education: Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing. B.S.N Preferred.Licensure: Current State License as an RN or LPN.Experience: Minimum of three years general nursing required with current home health, hospice, and medical, surgical or critical care experience preferred. Previous experience in home health, hospice, geriatrics, or general medical surgical nursing.Skills: Knowledge of clinical manifestations of diseases and ability to develop specific clinical knowledge. Understanding of specialty programs, and available services for eligible patients. Excellent communication, negotiation and public relations skills. Excellent presentation and public speaking skills.
