    Health and Wellness Nurse Job Overview Works with the Health and Wellness team and in collaboration with the Addiction Medicine team in the provision of medication education, support, and adherence, the treatment with Medication Assisted Therapy (MAT) for clients with Opiate Use Disorder (OUD) and other Substance Use Disorders, and coordinates care for the client with the residential behavioral health team, the addiction medicine team, primary care, and other internal and external healthcare resources for the client. Assists the addiction teams provider in the education, counseling and treatment with MAT under standing orders, provides health education around Substance Use Disorders and treatment, coaches and guides clients during the treatment process with Buprenorphine and other medication assisted treatments. Works with behavioral health staff in ensuring access and performs triage for clients seeking treatment for OUD and other substance use disorders. Collaborates with the healthcare team in any quality assurance or quality improvement in the provision of substance use disorder treatment. Works along with healthcare team in the integrative process of Substance Use Disorder treatment, Mental Health, primary care, and other healthcare services. Collaborates with the Health and Wellness team in medication storage, disposal, and control in the residential setting as well as providing client health education. Key Responsibilities Leadership Responsibilities: Works collaboratively with the Health and Wellness staff. Provides clinical leadership for client health education, client medication support, and within the quality improvement infrastructure of the healthcare and residential program. Participates in matters related to performance and quality improvement, planning, protocols and goal setting. Comfortable in changing systems, champions change, and lean process change. Organizational Responsibilities: Properly and consistently documents required information accurately on Clinic records and reports. Keeps up to date with operational and procedural requirements. In conjunction with Director of Addiction Medicine, assures organizational readiness for accreditation surveys and ongoing monitoring and reporting of conformance to quality within the clinics. Supports the interpretation of standards and processes to meet best practice standards established in the health services program. Coordinates and consults with the appropriate department heads within the agency to facilitate integration between programs. Provides direct care coordination support for clients to access all aspects of HR360 healthcare services. Serves on appropriate committees and work groups and other related committees of agency. Performance and Quality Improvement Responsibilities : In conjunction with the Director of Addiction Medicine, writes, reviews, and makes recommendations for policies and procedures for nursing or organizational processes regarding Health and Wellness in the residential setting. Active participant in Quality Improvement activities and may act as change champion or lead. In conjunction with the Director of Addiction Medicine, is responsible for working with external stakeholders on audits, reviews, or surveys and supports internal efforts as appropriate Training Responsibilities: Trains nursing students, nursing volunteers, health and wellness staff, and behavioral health staff to improve the clinical skills of staff. Orients new nurse staff, volunteers, and behavioral health staff to appropriate policies, procedures and protocols of the medical clinic. Supports staff training in quality improvement functions related to their specific position; e.g., Medication Assisted Treatment, basic customer service and client interaction skills; clinical documentation processes; incident reporting; confidentiality and privacy. Provides training on clinical and quality improvement issues, as needed and appropriate. Program Development and Project Management Responsibilities: Participates in the development of program areas that support nursing care, complex care management, care coordination, the client care team, and client continuity of care. Participates in grant/funding program development activities as appropriate Nursing Responsibilities: Manages medication storage, administration, and adherence for clients. Monitors patient medication use and helps to mitigate risk to patient by flagging high dose medications and potential medication interactions to the Director of Addiction Medicine. Also support Medication Assisted Treatment through counseling, education, and administration of injectable medications under the supervision of the addiction provider and standing orders. Provides appropriate client care education and counseling. Manages prescription refills under the guidance of the addiction provider and standing orders. Provides complex care and care coordination for all clients accessing healthcare services internal and external to HealthRIGHT 360.Works closely with staff collecting and reporting contractual data as necessary. Functions as a nurse within the scope of California BRN licensure and credentialing and privileging of the agency. And, other duties as assigned. Education and Knowledge, Skills and Abilities Education and Experience Required : BA or equivalent degree. Graduate of an accredited School of Nursing, Active California RN license, BLS Certification or must obtain within 30 days of hire. Desired: 1-2 years or equivalent experience, interest in working with homeless populations and in treating Opiate Use Disorder and other Substance Use Disorders Bi-lingual in Spanish or Cantonese Knowledge Required: -Culturally competent and able to work with a diverse population -Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications -Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self sufficiency - Proficient in language in addition to English - Experience in managing volunteers Preferred: - Experience with electronic medical records. - Comfort working on teams and with safety net populations. Skills and Abilities Required: - Ability to enter data into various electronic systems while maintaining the integrity and accuracy of the data - Professionalism, punctuality, flexibility and reliability are imperative -Excellent verbal, written, and interpersonal skills -Integrity to handle sensitive information in a confidential manner -Action oriented -Strong problem-solving skills -Excellent organization skills and ability to multitask and juggle multiple priorities -Outstanding ability to follow-through with tasks -Ability to work cooperatively and effectively as part of interdisciplinary team and independently assume responsibility -Strong initiative and enthusiasm and willingness to pitch in whenever needed -Able to communicate well at all levels of the organization including working with organization leadership and high-level representatives of partner organizations -Able to work within a frequently changing project scope while maintaining overall direction and structured priorities Desired: -Knowledge of co-occurring disorders and trauma informed treatment - Knowledge of Clinical documentation (treatment plans, progress notes etc.) -Experience working with safety net and criminal justice populations -Bilingual in Spanish Click here to apply for this job.
