    Parent Advocate/Case Navigator Job Overview The Prevention & Aftercare (PnA) program provides strength-based home visitation, case management and navigation services with parents who are at-risk of entering the child protection services system. This is a DCFS funded child abuse prevention program. The Parent Advocate/Case Navigator will provide outreach and engagement, community-based support services, home visitation, intensive case management, groups, parenting classes, and advocacy. Key Responsibilities Conduct outreach and engagement, recruitment, and enrolling of participants. Conduct needs assessment, identify strengths, and collaboratively develop a case plan to help participants define goals. Engage participants in ongoing follow-up and motivation to support their goals. Provide home visitation and support in areas such as home safety, parenting, life skills, goal setting, and other family strengthening activities. Provide intense Case Management including assistance with housing, benefits, employment, education, transportation, childcare, medical and other services. Empathetic support and feedback that is evidence-based, trauma informed, and culturally competent. Connect participants to social service and community resources and assist families in navigating systems. Provide advocacy and support for families with an open DCFS case to meet their goals and shorten the time line to family reunification and case closure. Co-facilitate and lead evidence-based parenting groups, anger management, domestic violence, and other groups. Maintain confidentiality in compliance with agency, HIPAA and DCFS standards. Responsible for maintenance of client charts and all contact documentation is completed in a timely and accurate manner. Education and Knowledge, Skills and Abilities Some College required, Bachelors degree preferred Minimum 3 years work experience in prevention, community based, field-based, or other social services working with families with complex needs. Two years experience (or life experience) working with the DCFS child protective services system Willingness to travel to provide field-based services Dependable automobile, California Driver's License, and valid car insurance. Must be able to pass background/criminal check. Bilingual in Spanish or Asian language preferred due to community needs in El Monte Click here to apply for this job.
