    Domestic Violence Case Navigator Job Overview The Prevention & Aftercare Services program provides strength-based home visitation, case management and navigation services with parents who are at-risk of entering the child protection services system. This is a DCFS funded child abuse prevention program. The Domestic Violence Case Navigator will provide direct services, education, information and referrals to survivors of domestic violence. Key Responsibilities Conduct outreach and engagement, recruitment of families, and enrolling of participants. Provide domestic violence prevention services through presentations and activities to build domestic violence awareness in the community. Conduct assessment, identify strengths, and collaboratively develop an Individualized Service Plan to help participants define goals; Reassess and revise service plan as needed. Assist client to develop a safety plan, assist with restraining order, and provide ongoing follow-up to help client keep their family safe from violence. Provide client advocacy, empowerment and motivation to follow their case plan and engage in behaviors that will prevent child exposure to family violence. Provide case navigation including making referrals, linking clients to supportive services, removing barriers to accessing services, and following-up on connections. Facilitate groups on domestic violence, healthy relationships, and nonviolent communication. Provide advocacy with DCFS for those with an open case. Maintain confidentiality in compliance with agency, HIPAA and DCFS standards. Responsible for maintenance of client charts and all contact documentation and ensure that all documentation is completed in a timely and accurate manner. Collect all pre and post surveys and outcome measures. Education and Knowledge, Skills and Abilities Bachelors Degree in psychology, social work, education, or other related field Must have 40-hour Domestic Violence Advocate Certification Minimum 3 years experience with high-risk families who have complex needs. Experience with evidence-based, culturally competent, gender responsive, and trauma informed services. Willingness to travel to provide field-based services Dependable automobile, California Driver's License, and valid car insurance. Click here to apply for this job.
