    Position Summary

    The Home Health amp; Community Based Services Manager oversees the operations of the Petersburg Medical Center's Home Health Agency and Adult Day Program. This role ensures high-quality, patient centered care aligned with PMC's mission, vales and regulatory standards. The manager collaborates with the medical staff, department leaders and community partners to support clients in maintaining health and independence at home.

    Core Responsibilities

    • Supervise and support registered nurses, aides, and other assigned home health staff.
    • Manage daily scheduling to ensure consistent service availability.
    • Conduct home assessments to evaluate client needs, including medical, functional, psychological and environmental factors.
    • Accept and manage referrals, confirming eligibility and initiating reimbursement processes.
    • Develop and oversee individualized plans of care in coordination with clients, families and physicians.
    • Perform skilled nursing tasks, medication administration, education and interdisciplinary referrals as needed.
    • Maintain accurate and compliant documentation in the medical record.
    • Oversee home health aide care planning and supervision.
    • Participate in Quality Improvement and Professional Advisory Committees.
    • Ensure policy and procedure manual is maintained and reviewed annually.
    • Provide clinical guidance and oversight to RNs and CNAs.
    • Collaborate with leadership to establish staffing patterns, budget needs and service standards.
    • Ensure physician involvement in plan-of-care development.
    • Establish clear delegation of authority and communication within the program.

    Working Conditions

    Environment. Variable; includes client homes, Adult Day program site, and other community settings.

    Hazards. May include driving in adverse weather, assisting clients with high care needs and managing behavioral health concerns.

    Qualifications amp; Education

    • Graduate of an accredited nursing program.
    • Current Alaska RN licensure.
    • 2-5 years of healthcare experience required; supervisory experience preferred.
    • Strong communication, leadership, and clinical judgement skills.
    • Knowledge of home health regulations and agency protocols.
