Medical Social Worker Department: Clinical Location: Marshalltown, IA Iowa Home Care has an opening for a Medical Social Worker to provide cares to individuals in their homes. Responsible for reducing or eliminating patient psycho-social stressors creating barriers to effective medical cares. Create patient-focused, strength and recovery-based medical social services. *Evaluate/assess psychosocial stressors affecting patients *Participate in development and implementation of care plan *Serve as resource to patients and clinical staff Requirements *Masters in Social Work from a school of social work accredited by the Council on Social Work Education *One year of social work experience in a health care setting *Valid Iowa driver's license *Knowledge of nursing care in accordance with regulations and practices established by the Iowa Board of Social Workers *Ability to assess client condition and implement plan of care, meeting physician order and client needs *Ability to maintain a valid professional licensure *Excellent communication skills *Previous experience in social work *Knowledge of community resources *Strong case management skills *Previous experience with geriatrics and/or persons with mental illness is preferred Please send your resume to humanresources@iowahomecare.com or apply online at www.iowahomecare.com.
