Full Time 11a-11P Job Summary: Under general supervision, The roles responsibilities are to meet with patient and families in the Emergency Department to provide a real-time, primary care follow-up appointment with a KentuckyOne Health provider. The Patient Resource Scheduler will be responsible for coordinating with physicians offices to assure a smooth transition to a KentuckyOne Health or relevant provider. The Patient Resource Scheduler will be responsible for data collection, updating changes to office guidelines. This staff group will also be responsible for collaborating with the Communications team to cover as needed. Job Responsibilities: Identify emergency department patients with no primary care provider Meet with these patients as part of the patient discharge process to offer a scheduled appointment with a primary care provider Determine appropriate primary care provider based on patient demographics Direct schedule into Allscripts for any patient following up with KYOne Health Medical Group (KMG) provider Follow scheduling rules as outlined by KMG and any other third party where patients may be referred (such as federally qualified health center) Track patient activity in format and to level of detail required by KYOne Participate in daily huddles and/or other meetings as required by management Follow up on scheduled appointments to determine status (no show, cancelled, arrived) Perform other duties as assigned. Experience (required and preferred): At least 2 years of customer service and/or scheduling experience in a healthcare setting preferred. Education : High School diploma required. Skills, Knowledge or Abilities critical to this role: Language Ability : Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to common inquiries or complaints from groups of managers, clients, customers, and the general public. Math Ability : Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Reasoning Ability : Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills : Must be proficient with the Microsoft Office Suite, Internet navigation, database management, and, spreadsheets.
