These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health and other employed team members. This role will be responsible for raising awareness about the Landmark Program in the local community, with an emphasis on educating patients who are eligible, but not currently using our services. Responsibilities include working with Community Health Advocates to learn and utilize local strategy and tactics to reach the member population and taking direction and coaching from the General Manager and Regional Medical Director on PCP and provider outreach. This role is responsible for regular reporting on production and progress. Responsibilities Overview of Job DutiesField-based and telephonic outreach to patients, including door-to-door campaigns, to reach vulnerable patients so that we can notify them about their eligibility and assist them in enrolling in the Program.Outreach to primary care clinics, assisted living/long-term care facilities, and hospitals to educate them about the Landmark Program and to coordinate with them to improve patient engagement.Relationship-building with key community resources, such as home health and hospice agencies, senior day care centers, and more.Maintain and ensure the confidentiality of all Protected Health InformationDocument and track patient outreach activity.
