    Horizon Health is seeking two Full-Time Licensed Social Workers in any Horizon Health location or remotely in the state of South Dakota. The Licensed Social Worker acts as a member of the primary care team to help patient with goal setting in attempt to improve health outcomes and will provide counseling and enabling services to patients within clinic setting. Successful candidates for the Licensed Social Worker should be passionate about providing high-quality, patient centered care.     • Eligible candidates must be a Certified Social Worker with a Private Independent Practice (PIP) to practice in the state of South Dakota. Horizon Health will provide required PIP supervision and cover the cost of the PIP certification exam for the right candidate.

    Key Responsibilities:

    Conduct intake screenings to evaluate the needs of patients, provide counseling, perform brief visits, and provide enabling services/case management for patients served as identified by organizational standards.

    Collaborate with PCPs and clinic support staff organizationally using brief model to help patients set goals that align with treatment plan and provide support to attain improved health outcomes for patients.

    Engage in the overall behavior health program within the organization to facilitate growth and process improvement to meet the needs of the patients and to align with the organizational mission.
