Social Work Intern PRN - Family Medicine Under the general

supervision,collaborates with Residents, Pharmacists, Behavioral Health Consultants, andoutside community and state agencies in the coordination of patient care,assesses patients to identify and reduce barriers to the access of health care,nutrition, transportation, and other social needs, research services andresources that are available to the patients within the organization and thesurrounding community, make referrals to appropriate resources, monitorspatient progress toward goals, and evaluates intervention outcomes. Minimum Education: High school diploma or equivalent. Must be currently enrolled in a SocialWork or Counseling program. Minimum Experience: Knowledge about any ofthe following areas: marriage, speech, sociology, personal health, humanbiology or casework methods.
