(Eligibility based on FTE) PHYSICAL THERAPISTS & PHYSICAL THERAPIST ASSISTANTS! JOIN THE LINDENGROVE FAMILY AND MAKE A DIFFERENCE IN THE LIVES OF OTHERS Duties: Provides treatment and evaluation upon physician referral according to the American Physical Therapy Association and facility standards/procedures. Provides comprehensive management and treatment to clients to improve quality of life. This is achieved via by assisting in the improvement of and/or the prevention of decline in functional ADL, motor and cognitive skills. These skills may be impaired by disease, developmental delay, aging process or trauma. Plans long and short term goals appropriately based on the evaluation and resident needs to improve functional skills. Collaborates with resident and significant other in discussing the evaluation, goal development, treatment procedures, and progress. Uses reason when planning frequency and duration of treatment for residents on caseload. Modifies and updates treatment methods when necessary. Re-evaluate treatment plans accurately at appropriate intervals. Reinforces resident to enhance rapport/treatment outcomes. Discontinue therapy as indicated. Establishes home/unit programs when appropriate. Supervisory Responsibilities for Physical Therapists: Directly supervises Rehabilitation Assistants and Physical Therapist Assistants. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Requirements for Phyical Therapist Assistants: Education: Graduate of an approved Physical Therapist Assistant training program (two year Associate Degree). Experience: Completion of a clinical affiliation in an accredited facility and one month of on-the-job exposure. LindenGrove Communities
