Primary Clinician
Great location in Milford - approx. 30 minutes from Worcester, Framingham, and Woonsocket RI
We are seeking a Primary Clinician for our Adult Community Clinical Services (ACCS) program!
As the primary Clinician, the candidate will work to establish a therapeutic relationship, and focus on skill building to support the person in maintaining independent housing and community tenure that is uninterrupted by hospitalizations.
The Clinician provides care coordination through development of a collaborative relationship with the person receiving services, family, and other providers and community supports that are important to the person. The Clinician completes person-centered assessment and service planning, advocacy, coordination of care, and helps the person to make connections with other services and resources within the community.

We can now hire Occupational Therapists (OTR/L) registered by the National OT credentialing board with Massachusetts Licensure for this role!
Pay Rate: $72,100 salary unlicensed/LCSW; OR $75,189 salary independently licensed (LMHC or LICSW)
Schedule: 40 hours, Monday through Friday

Required Skills
Required Experience
Valid license as Registered Nurse in the state of Connecticut.
ASN from an accredited School of Nursing required. BSN preferred; will consider candidates with a plan to pursue BSN or BSN in progress.
BLS certification required.
Minimum of one-year nursing experience required.
