     ABOUT THE POSITION:  

    The Care Center is a team of Care Coordinators, and Clinical Outreach Specialists (LPNs & RNs) who support Pre and Post Assessment activities. The team is often the first and/or last touch with the member; establishing rapport and leaving a positive member experience.

    Using a member-centered approach, the Clinical Outreach Specialist engages with patients before and after they have completed a Health Assessment by a Matrix Network healthcare professional while in their home, on a Mobile Health Clinic, via telehealth visit, or in the employee health and wellness setting. This critical role conducts telephonic outreach to gather responses to pre-assessment questions and help the member prepare for their upcoming visit. Post-assessment, the Clinical Outreach Specialist is also responsible for following up with members to provide lab results, address questions/concerns, and provide guidance on follow up critical for the next step in the member’s care.

    RESPONSIBILITIES

    • Become the clinical operations subject matter expert for the physician network. Be the primary point person for internal questions and requests related to the physician network.
    • Be the primary point person for the physicians themselves and establish a collegial relationship with them.
    • Help lead initiatives, and guide strategy, to expand and enhance the network and physician-related programs.
    • Work closely with internal stakeholders to ensure the success and growth of physician-related programs.  This includes legal, regulatory, operations, human resources, and executive leadership.
    • Ensure Matrix has the ability to meet it’s evolving physician-related demands as a growing and rapidly evolving organization (eg. Enhanced clinical services, prescriptive authority, etc).
    • Work with appropriate subject matter experts to ensure physician-related programs comply with national and state regulatory requirements.
    • Coordinate on administrative tasks with other business units (eg. Credentialing Team/HR) such as:
      • Manage monthly chart review as required by state regulations. Includes ensuring successful delivery and continued follow up through completion.
      • Managing the process of onboarding new physicians and the process for physicians leaving the company
      • Managing physician capacity as outlined by state regulations   
      • Ongoing evaluation of state collaborating physician requirements and chart review process
      • Creation and delivery of informational/educational material as required
      • Working directly with clinical team on creation and maintenance of documentation as outlined by individual state requirements
    • Seek to enhance and ensure physician satisfaction.
    • Establish and support programs to ensure the quality of physician-provided services.
    • Coordination of regular Physician Network Meetings (eg. The Quarterly Collaborating Physicians Meeting)
    • Performs other duties as assigned

    QUALIFICATIONS

    Educational Requirements:

    • Current unrestricted licensure to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state(s) to which the candidate is assigned
    • Maintains nursing license in good standing

    Required Skills and Abilities:

    • Ability to provide telephonic support and education to members in a way they are able to understand
    • Exceptional telephonic communication skills with health plan members or caregivers
    • Ability to build rapport and maintain positive, pleasant demeanor with all levels of internal and external contacts
    • Maintain professionalism with members, caregivers, and other health care team members at all times  
    • Consistent and effective follow-up skills
    • Effective written communication skills
    • Ability to prioritize activities in a rapidly changing environment
    • Proven time management and organizational skills
    • Professional telephone and email etiquette
    • Being comfortable with phone conversation, email, and fax as the main means of communication
    • Ability to effectively work within a group dynamic
    • Possess unquestionable personal and professional ethics, in concert with company standards
    • Ability to work independently, coordinating, and assimilating multiple pieces of data to accomplish required tasks
    • Ability to work in diverse, multi-cultural environment and be respectful to health plan members’ cultural and socioeconomic characteristics
    • Proficiency in Microsoft Office (Word, Outlook, Excel) and the internet
    • Ability to assess when escalation of items is necessary and escalate to the appropriate resource

    Experience:

    • Minimum three (3) years of nursing experience
    • Home care experience preferred (not required)
    • Telephone triage experience preferred
    • Must have experience working with the Microsoft Office Suite (Outlook, Word, Excel)

    Supervisory Responsibility: No supervisory responsibilities

    Travel Requirements: No travel necessary

    Work Conditions: General office environment
