    Mental Health Resource Center is a not-for-profit Florida corporation that provides a wide range of mental health and behavioral health care services to the community such as 24-hour emergency services, inpatient psychiatric services for children, adolescents, and adults as well as outpatient services such as medication management, case management, and counseling.

    Job Summary

    The goal of the Housing Specialist is to assure that people with behavioral health needs have access to safe and affordable housing, with a goal of maximizing community capacity. This is accomplished through carrying out assigned work duties within the assigned region to facilitate the development of/access to safe and affordable housing for adults with mental illness, intellectual/developmental disabilities and substance use/addiction disorders. The goal is to increase the number of options within a community-based system of supportive services, allowing individuals to remain in and/or transition to the community and to have access to behavioral healthcare services and affordable housing. The Housing Specialist will work in collaboration with the MHRC Care Transitions Team, Care Coordination Outreach Team and other community-based programs, agencies, governmental entities, and faith-based organizations to promote the development of safe and affordable housing. This position will work to develop and leverage new housing resources for individuals in need.

    Responsibilities

    Participates as an integral and essential part of the Care Transitions Team and Care Coordination Outreach Team (inclusive of attending Care Transition and Care Coordination Meetings). This includes facilitating the supportive housing component that assists individuals in securing homes in their community of choice; this may include assisting participants directly in securing housing as well as working with provider agencies to achieve this outcome.

    In collaboration with the Care Transitions Team and Care Coordination Outreach Team, the Housing Specialist serves as the primary contact with community housing programs and agencies to collect information and resources to best access services.

    Actively promotes the identification and/or development of safe and affordable housing options for individuals with behavioral health needs.

    Serves as housing expert educating and supporting MHRC employees, network providers, individuals and their family members, property owners/landlords and community stakeholders on housing resources and supports available and provides guidance and direction on how to access these resources.

    Collaborates and consults with local stakeholders and other affordable housing advocates for the purpose of developing connections to maximize existing affordable housing resources and identify strategies to develop new resources.

    Collection and Quality Assurance of Data for reporting purposes as assigned by Supervisor.

    Gathers supporting documentation for initiatives upon request by Supervisor or other MHRC staff (with Supervisor approval).

    Participate in housing support efforts/committees as assigned by Supervisor.

    Keeps Supervisors informed of regional housing trends.

    Ensures timely and thorough documentation of all efforts, both individual-specific and programmatically, as requested by Supervisor, and the Care Transition and Care Coordination Teams.

    Engagement with person served and their natural supports ‐ the Housing Specialist goes to the individual and builds trust and rapport. The Housing Specialist actively seeks out and encourages the full participation of the individual’s networks of interpersonal and community relationships. The care plan reflects activities and interventions that draw on sources of natural support.

    Provides community‐based – services and supports in the most inclusive, most responsive, most accessible, and least restrictive settings possible that safely promote an individual’s integration into home and community life.

    Advocates for acquisition of the services and resources necessary to implement facilitate access to safe and affordable housing. Completes referrals to community services and resources as needed. Coordinates the delivery of services as they pertain to housing transitions. Monitors and evaluates effectiveness and satisfaction with services.

    Qualifications

    In order to be considered, a candidate must have a Bachelor's Degree in Social Work or a related Human Services field from an accredited university or college (a related Human Services field is defined as one in which 30 hours of course work includes the study of human behavior and development) required.

    One year experience working in human services or mental health related field required.

    Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System demonstrated within three months of employment.

    Proficiency in Microsoft Office, Outlook and use of the Internet required.

    Must meet Frequent Drivers requirements, including a valid Florida driver’s license, and insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits.

    Requires the ability to travel to satellite facilities, community agencies, and to make contact with individuals by performing home visits or community outreach.

    Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including patients, families, caregivers, community service providers, supervisory staff and other department professionals.
