    Job Description

    Interested in joining the COVID fight and supporting early intervention, testing, and education of a typically underserved population?

    MNHC is seeking a detailed, bilingual (English/Spanish) COVID-19 Contact Tracer to join a patient-focused, primary care-centered COVID-19 Testing team at a large inter-disciplinary community health center in San Francisco’s Mission District.

    Key duties: 

    • Act as primary point person to call patients with a positive COVID-19 test. Provide support and gather information. Interview and provide support to close contacts within the households.
    • Communicate with patients in a cultural and linguistic appropriate, professional, and empathetic manner
    • Collect and record information from interview with cases into system in timely manner.
    • Provide cases with information about isolation and quarantine, based on San Francisco Department of Public Health (SFDPH) guidelines. If needed, refer to SFDPH Isolation and Quarantine unit.
    • Escalate issues to Lead RN and Nurse Manager.
    • Maintain daily communication with Lead RN.
    • Work collaboratively with RNs, Community Health Worker, COVID-19 Testing Assistants.
    • Screen patients for necessary community resources (i.e. emergency food, rental assistance, legal aid). Refer patients to Community Health Worker for follow-up.
    • Participate in daily team Huddles and monthly COVID-19 Testing Team meeting.
    • Maintains confidentiality of all patient information in accordance with federal guidelines and regulations.

     

    Job Requirements:

    • Bilingual English/Spanish REQUIRED. Fluency in other languages preferred.
    • High school diploma or equivalent required.
    • Experience in health, human service, or community-based organization or setting strongly preferred
    • Strong interpersonal skills required. Ability to show empathy and interact professionally with culturally diverse individuals and communities during a time of crisis, distress, and homelessness
