New Horizon's Counseling Center ACT Team serves individuals with mental health conditions who are homeless and temporarily housed within a Queens's mental health shelter. Team members work collaboratively with shelter staff and use assertive engagement strategies to proactively engage individuals in services. ACT team members share responsibility for the people they serve, helping individuals attain housing and other personal goals that may be of a health, social, work or education nature.

POSITION DUTIES INCLUDE:

· Monitoring treatment and the delivery of clinical services

· Assisting the psychiatrist in psychiatric and medical assessment and treatment, clinical supervision, education and training of the team and development, maintenance and supervision of medication administration and psychiatric and medical treatment and procedures

· When not performing psychiatric functions, the PMHNP may assist the Registered Nurse functions of monitoring overall medical and psychiatric health, providing education to client regarding their care, administering medication etc.

Psychiatric Nurse Practitioner must have experience working with SPMI/SUD adults and licensed by the NYS Education Department.

QUALIFICATIONS

• Valid New York state license for practice as a Psychiatric Nurse Practitioner;
• Extensive experience working with people with serious mental illness, and co-occurring substance use disorders, and primary health care problems
• Experience working with the justice-involved and homeless population with mental illness and knowledgeable about risk assessment and risk management
• Excellent written and communication skills.
