JOB DESCRIPTION
As a member of an interdisciplinary team, the Behavioral Health Specialist a primary counselor and case manager.
Responsible for psychosocial assessment, treatment planning and providing direct support for clients on caseload
Using a person-centered approach to engagement, mental illness and recovery, the Behavioral specialist supports consumers to develop wellness plans and supportive healthy community networks to access community resources.
The Behavioral health specialist provides services in the community within the framework of core ACT operating principles, and will take the lead to get recipients access to self-help resources and networks.
Coordinates psychiatric and medical treatment, as required. Provides concrete social service assessment and interventions.
Educates patients and families as appropriate to patient needs.
Participates in conferences, training programs, quality assessment and improvement activities, departmental, discipline and team meetings.
Prepares and maintains appropriate records, summaries and reports following the guidelines of assigned unit.
The Behavioral Health Specialist must be a team player with knowledge of psychiatric disability, recovery, and the special treatment needs of consumers with serious mental illness.
Provide individual support and wellness counseling, and harm reduction counseling based on motivational interviewing
Work effectively with the consumer's support system, and self-help groups providing services to ACT consumers.
Link consumers to peer support and self-help recovery resources
Complete face-to-face treatment contacts in the community every month with every consumer on the ACT team
• Act as advocate and liaison for consumers in income support and access to resources to support service plan goals
• Involve family, significant others, and housing providers in ACT treatment
Provide 80 percent of treatment contacts in the community
