But every minute, they're always Putting Patients First. nbsp; Now, you can make it possible. nbsp; In this role you will support the Ambulatory Care Management team with a variety of activities. Assist with interdisciplinary team rounds to bridge the gap between the various disciplines while supporting a continuum of care for our patients. During rounds you will scribe minutes and provide a summary on all outstanding action items to ensure safe patient outcomes.nbsp; Use Excel, Allscripts and other systems to track census, and prepare data on patient trends. Communicate with Providers, Social Workers, Care Managers, and RNs.nbsp; Here, youll experience all the reasons why you entered such a rewarding field.
