Assist with interdisciplinary team rounds to bridge the gap between the various disciplines while supporting a continuum of care for our patients. During rounds you will scribe minutes and provide a summary on all outstanding action items to ensure safe patient outcomes.nbsp; Use Excel, Allscripts and other systems to track census, and prepare data on patient trends. Communicate with Providers, Social Workers, Care Managers, and RNs.nbsp; Here, youll experience all the reasons why you entered such a rewarding field.
