Social Worker NorCal Care Coordinator Agency - Citrus Heights, CA $35 - $50 an hour - Part-time Norcal Care Coordinator Agency is a care management company that specializes in the placement of Medi-Cal eligible patients into assisted living facilities under the Medi-Cal ALWP guidelines. The Medi-Cal Assisted Living Waiver Program is designed to assist Medi-Cal beneficiaries to remain in their community as an alternative to residing in a licensed health care facility. The program provides specified benefits to eligible seniors and persons with disabilities. The Social Worker/Case Manager is an integral part of the Case Management team. The team is responsible for developing a plan to meet the clients' care needs by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring services provided. Minimum Requirements. Bachelors or Master's degree in a social work, psychology, counseling, rehabilitation, gerontology or sociology from an accredited college or university. At a minimum, one year of experience must be in a social services field in a relevant position. Able to instruct, maintain positive community relationships, effectively interface with participants and staff, and motivate participants to create an atmosphere of enthusiasm and a sense of work ethic among participants. Able to write clear and concise reports. Ability to work with computers and the necessary software.
