Position Overview ** **We are currently seeking a Social Worker for our Care Management/Social Services departments located at both Glenbrook and Evanston hospitals. This is a full-time position and is benefits eligible. Hours will be scheduled 8:30AM-5:00PM, Monday-Friday. Will be required to work a weekend every 6 weeks. Must be able to work at both Glenbrook and Evanston hospital sites. ____ Responsibilities **** In this role, you will: * Conduct and document assessment of patient and family needs, communicate care issues, and develop appropriate plan * Provide counseling and crisis intervention on an individual or group basis * Patient advocacy including but not limited to education of patient and family rights, case management services, and staff education regarding identification and reporting of abuse/neglect
