Job Description New York City Health and Hospitals is seeking a dedicated and highly motivated Clinical Pharmacist for a Cardiology/Internal Medicine/Transitions in Care Position for Department of Pharmacy at Lincoln Hospital. This full-time faculty position will have a responsibilities including implementation of the CHF pathway in collaboration with a clincial coordinator and who's role will include development of a pharmacy practice site, rounding, early identification and prevention of adverse drug reactions and interactions, rounding, early identification and prevention of adverse drug reactions and interactions, therapeutic drug monitoring, medication reconciliation, and promoting medication adherence. Specifically the pharmacist role in heart failure pathway will include: 1.Being part of a multidisciplinary team caring for patients with cardiovascular disease in general and HF in particular. 2.The clinical pharmacist activities will include clarifying drug orders and identificaiton and resolution of drug-related problems, leading to fewer mediciation errors and adverse events. 3. Reducing overall healthcare expenditure related to inpatient management of HF patients and pervention of preventable adverse events. 4. Participate in focused rounds related to CHF pathway and perform pharmacy interventions such as: * Drug information, Therapeutic consultation, Order clarification and formulary maintenance, Drug interaction avoidance, Renal dose adjustments and Dupications of therapy and Quality improvement. 5. The clinical pharmacist will assure four core measures are documented such as: * Assessment of left ventricular ejection fraction, angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) use in patients with left ventricular systolic dysfunction, Documentation that patients receivied discharge instructions, and adult smoking cessation counseling. Minimum Qualifications For Level I: 1. Possession of a valid NY State license to practice pharmacy. For Level II: In addition to meeting the qualification requirements of the lower level: 1. Two years of post licensure experience; or 2. Completion of a post graduate specialty residency or fellowship program; or 3. Current Pharmacy Board Certification. For Level III: In addition to meeting the qualification requirements of the lower levels, an additional: 1. One year of post licensure supervisory experience or specialty clinical experience. Department Preferences Qualified candidates must possess a Doctor of Pharmacy degree from an ACPE-accredited institution, be eligible for licensure in the New York, and have completed a PGY I residency a PGY 2 or Fellowship in Cardiology pharmacotherapy or transition of care would be highly considered, or equivalent trainings or experience (at least 2 years. Board certification preferred. Candidate must posses a strong commitment to practice, teaching and scholarship.
