Psych Social Worker MSW Req #: 30111513 Category: Clinical/Allied Health Facility: Beaumont Hospital FH Department: Care Management Schedule: Full time Shift: Afternoons/Evenings Hours: Varies Job Details: General Summary: Provides crisis therapy, appropriate pre- and post-hospital referrals directed towards ameliorating patients, families and/or significant others psychosocial dysfunction. Provides appropriate in/outpatient psychosocial assessment and follow-up, counseling and teaching adaptive coping mechanisms. Facilitates necessary communication between patients, families, interdisciplinary team and the community while demonstrating clinical knowledge of the psychosocial aspects of disease process, signs and symptoms. Communicates and coordinates patient's continuing care needs and discharge plans with Professional Staff, health care professionals, patients and families. Assists the team in ensuring excellent patient care, coordination of educational initiatives, providing outreach services and linking team efforts with other hospital departments, outpatient and community services. Maintains current knowledge and social work practice skills relevant to intervention, modalities, community resources, legal mandates and codes, Hospital and Department policies and procedures. Essential Duties: * Assesses and documents patients referred to Continuing Care for extended care placement (e.g., hospice, nursing home, rehabilitation, foster home and shelter). * Formulates, documents, implements and evaluates patient's continuing care plans. * Performs direct short-term problem solving and counseling, focused on identified problems. Assesses patients and families reception to social work and community support services (e.g., guardianship, abuse/neglect, appropriate community resources) and proceeds with counseling accordingly. Documents intervention, outcomes and follow-up. * Provides crisis intervention (within fifteen minutes of receiving the call or referral) in the Emergency Department. Initiates appropriate contacts with the Professional and Nursing Staff, Hospital Ancillary Staff and Community Agencies. Documents intervention outcomes and follow-up. * Monitors therapeutic interventions and regimens of the oncology patient population. Maintains an effective therapeutic relationship with patient/family as demonstrated by professional boundaries, identifying transference and counter transference issues, expression of cultural sensitivity and patient/family responses. * Promotes patient self-determination by keeping patient informed of choices. Encourages patient communication of his/her wishes to the healthcare team. Communicates consistently in a manner that demonstrates a positive and cooperative attitude and maintains clear role expectations. * Completes documentation of Interdisciplinary Discharge Plan, accurately reflecting the patient's current condition, situational factors, transition care needs and discusses plan with physician, patient and/or family. Documents assessment, pertinent psychosocial factors, discharge plans and referrals. · Participates in hospital wide education efforts involving interdisciplinary teams and physicians. · Complies with Hospital Principles, Hospitality Rules, Department Goals and other Department programs and policies. · Performs Quality Assessment and improvements activities as directed. · Maintains knowledge of and utilizes current information regarding appropriate community resources, legal codes and mandates. · Performs appropriate liaison and public relations functions to enhance hospital services and community awareness including participating in appropriate meetings and committees. · Consults with the Manager, Case Management concerning policies, procedures and care problems and assists in planning, implementing and evaluating social work policies and procedures. · Performs and maintains accurate case records and case data. · Establish priorities among cases and assignments. · Educates Professional Staff and appropriate Hospital personnel to continuing care activities. · Provides employee assistance counseling and referrals as necessary. Standard Qualifications: * Education/Training: Graduate from an accredited Graduate School of Social Work. Master's Degree in Social Work required. * Work Experience: Minimum of one to two years Medical Social Work employment in an acute care facility. Experience with hospital or end-of-life issues highly desired for oncology assignments. * Communication and Contacts: Frequently interacts with patients, families, Professional Staff, health care professionals and community agency members. Contacts with the patients and families are often done in crisis situations. Medical Social Workers must have interpersonal skills necessary to interact with patients and families during times of crisis, high stress levels and agitation, always demonstrating a cooperative, helpful, caring and positive attitude. Medical Social Worker must be able to demonstrate courtesy, professionalism and a respect for individual values and preferences. * Analytical Demands: Requires the analytical ability necessary to provide comprehensive assessments of emotional, behavioral, environmental, financial and individual needs. Counseling efforts must be individualized. Knowledge of interviewing techniques, time management, problem solving, counseling and advocacy skills is necessary. Must have working knowledge of medical terminology. * Computer Skills: Keyboarding proficiency, ability to utilize hospital information and electronic medical records software/systems.
