Bachelor's degree in social services field or related field preferred. Two (2) years of experience in a social service or educational setting. Head Start experience preferred. Assists with completion of the application/enrollment process with participating families. Assists with the completion of social histories, nutrition assessments, and other health forms. Makes follow-up with community referrals to ensure eligibility to participate in the program. Ensures files are complete and updated. Assists with providing extensive case management services required by families involved in Head Start to include: ensuring that enrollees have access to needed services. Ensures all clients enrolled have access to medical care. Develops and implements follow-up plans. Works with parents to promote parent participation in the delivery of services for their children. Assists with recruitment of families with children ages three (3) to five (5). Provides information to the community about the program as assigned. Assists with the completion of social histories. Assists with the timely completion of professional, accurate, and thorough progress reports. Assists participants in locating and enrolling in life skills training classes/workshops that will meet their particular needs. Makes home visits to participating children/families as assigned. Keeps Coordinator and Site Manager informed of programmatic issues in a timely and concise manner. Performs research/study to remain current of the trends involving early childhood education, the needs of children at risk, and services to families affected by substance abuse. Contributes to team efforts by performing other duties as assigned. Physical Demands While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee frequently is required to use hands to finger/handle/feel. Occasionally required to stand and walk. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision. Work in a non-smoking environment. Special Conditions Maintain a valid driver's license with a good driving record with appropriate level of insurance.
