ResponsibilitiesSocial Worker - Emergency Services Providence is calling an Social Worker (0.8 FTE, Evenings) to join the Care Management team in the Emergency Department at Providence Milwaukie Hospital in Milwaukie, OR. The Social Worker in this position is a member of the multidisciplinary team that helps support people entering our emergency department with behavioral health needs. The schedule for this position is 32 hours a week, Wednesday through Saturday from 3pm to 11:30pm. This position is consider an exempt full time position with salary based pay and includes benefits and paid time off. In this position, you will: ** * Under direct and continuous supervision, provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients, and their support systems. * Incorporate concepts and theories associated with life span development, neurobiology, and behavioral change. * Collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT). * Perform duties in a manner that respects patient self-determination that reduces health disparities/barriers to care and adheres to the National Association of Social Workers (NASW) code of ethics, Standards of Practice (SOP), and mission. * Social Workers at Providence Milwaukie Hospital have a dual role. They are responsible for the transitional planning for all patients on the medical surgical floor and the Intensive Care Unit. This includes discharge planning, placements, family care conferences, palliative care and hospice and coordinating with community partners. They are also responsible for behavioral health assessments and transition planning in the Emergency Department. Must be able to shift roles fluidly, be excellent at time management and enjoy working in a chaotic environment. Required qualifications for this position include: * Masters degree in Social Work from a Council of Social Work Education (CSWE) accredited school of social work, or from a school in candidacy status by the CSWE * Temporary Clinical Social Work Associates (CSWA) license, or proof of CSWA initiated within 6 months of hire * Documentation of an approved supervision plan from the Oregon Board * Must obtain LCSW Board licensure within a maximum of 5 years from CSWA application, or date of hire date as a CSWA * 1 year Case management and/or medical social work * 1 year Behavioral Health crisis intervention Preferred qualifications for this position include: * Licensed Clinical Social Worker (LCSW) * Healthcare experience * 1 year experience in acute care settings ** About Providence in Oregon As the largest healthcare system and largest private employer in Oregon, Providence offers exceptional work environments and unparalleled career opportunities.
