    Clinical Nurse Leader - Spinal Cord Innovation Center, Part-time Tracking Code 41465 Job Description General Summary The Clinical Nurse Leader is a micro-system outcomes manager for a specified cohort of patients who collaborates with other healthcare disciplines, research and administration teams to manage, maintain and improve clinical, functional, cost and satisfaction outcomes. By utilizing the nursing process (Assessment, Diagnosis, Planning, Implementation, Evaluation) the Clinical Nurse Leader facilitates a partnership between the education and practice arenas in order to provide the highest quality of care for its patients and families. The Clinical Nurse Leader serves as a leader through consultation, collaboration and application of the nursing process with a focus on improving outcomes at the unit level. The Clinical Nurse Leader will consistently demonstrate support of the SRAlab statement of Vision, Mission and Core Values by striving for excellence, contributing to the team efforts and showing respect and compassion for patients and their families, fellow employees, and all others with whom there is contact at or in the interest of the institute. Principal Responsibilities Clinician Assessment Identifies and utilizes appropriate evidence-based techniques and instruments to collect pertinent physiological, psychosocial, and functional data to formulate nursing diagnosis. Interprets data from all health team members and integrates it into written assessment and formulates a nursing diagnosis based on available assessment data. Planning - Develops and modifies plan to attain expected goals/outcomes that is consistent with interdisciplinary plan of care and patient/family goals. Demonstrates respect, courtesy, understanding and tolerance while accomplishing shared goals. Communicates plan to patient and family and seeks input related to goal attainment. Implementation/Evaluation Implements interventions consistent with the established plan of care, interdisciplinary treatment plan, and available resources and evaluates progress toward goal attainment modifying the plan of care as indicated. Implements interdisciplinary treatment activities and evaluates patient level of performance and outcomes. Partners with interdisciplinary team to address complex patient/family goals and collaborates with Allied Health staff on interventions to improve performance and outcomes. Provides comprehensive nursing care for assigned patients by maintaining a current care plan and adjusting it to the patient's changing needs. Nursing care includes performing specializing treatments, administering prescribed medications, delegating duties to appropriate persons and coordinating patient care with other departments. Demonstrates competency in understanding the growth and development of infant, child, adolescent, young adult, middle-aged adult and geriatric patients as measured by specific performance standards and as described by department/unit policies and procedures. Demonstrates the ability to interpret growth and development related information to assure patient needs are met. Collects and reports pertinent physiological, psychological, and functional data based on patient plan of care or patient need. Documents medication, administration, functional care, intake and output, vital signs and other measure in RICIS or utilizes downtime procedures. Outcomes Manager Collects, analyzes nursing sensitive outcome data and collaborates with manager to develop plan to improve performance. Collaborates with manager, clinical nursing instructor and AH leaders to develop strategies to improve performance at unit level. Monitors patient satisfaction and preparation for discharge scores and works with nursing staff to improve levels of performance. Client Advocate Assumes accountability for a case load of patients, coordinates effective handoff of nursing care, and solicits feedback from nurses across the continuum or system care to evaluate outcomes. Assumes accountability for orienting patient/family to SRAlab, and identifying/evaluating high risk home discharges and coordinates follow-up phone calls. Educator Modeling/Role -modeling listening skills that provide a supportive caring environment and promote goal attainment. Reviews and critiques care plans for patients on the unit and works with staff to customize interventions to attain patient family goals and reduce the risk of complications during the rehabilitation stay. Identifies and develops plan to address learning needs of patients and families by providing accurate information regarding care during hospitalization including expected outcomes, home care instructions and referrals to appropriate health agencies and resources. Information Manager Identifies gaps in current available data and develops plan to collect, interpret, analyze, and use data to improve outcomes. Shares information with staff and team related to program performance and assists with development of action plans to address problematic areas. Reviews, evaluates and makes recommendations regarding Institute policies and procedures to insure they are evidence-based and bring most relevant research to clinical practice. Systems analyst/risk anticipator Safety anticipates, identifies factors related to safety for patients, visitors and staff and develops and implements plans to ensure said safety across the continuum of care. Team Manager Responsible for establishing and maintaining learning environment by working effectively with health team members, assisting with care and documentation and initiating and conducting patient care conferences with staff. Communicates patient care plan and patient interdisciplinary goal attainment and identifies when changes are needed. Assumes charge nurse responsibilities as assigned which include working effectively with team members and resolving interdepartmental problems and barriers to provision of necessary services to patients. Professional Development Professionalism and Development Evaluates own performance in relation to institutional practice standards, quality improvement initiatives, and institutional policies and procedures. Participates and shares information gained from involvement in professional nursing organizations and/or external nursing committees with staff. Contributes to the development of new nursing knowledge through publications, presentations or leadership activities. Perform all other duties that may be assigned in the best interest of SRAlab. Reporting Relationships Reports directly to the Nurse Manager. Knowledge, Skills & Abilities Required Minimum education requirement of a Masters of Science degree (or equivalent) from an accredited nursing program. Current Illinois Registered Professional Nurse license. National certification as Clinical Nurse Leader, or completion of CNL within one year, and ongoing maintenance of Clinical Nurse Leader national certification/annual continuing education requirements. Current CPR certification. Two years relevant nursing experience preferred. Demonstrated track record of success in leadership roles and initiatives. Exceptional organization, communication and interpersonal skills. Excellent problem solving abilities. Communication - Ability to communicate effectively using timely verbal, non-verbal and written communication. Accountability - Ability to proactively take ownership of serving the customer, team, and organization, of achieving goals and of resolving problems. Ability to consistently do whatever it takes to ensure individual, departmental/team and organizational excellence. Flexibility/Adaptability - Ability to adapt willingly to changing work demands and work environment. Judgment/Problem Solving - Demonstrated critical thinking skills and ability to make sound judgments and to monitor and respond to patient needs or changes in condition. Customer Service - Ability to anticipate, assess, and address the needs of patients, internal and external customers to positively influence the patient/customer experience (directly and indirectly). Ability to transfer and position patients consistent with lifting 30 lbs.
