Care Overview Thenbsp;Care Coordinatornbsp;provides strength-based,
culturally competent Wraparound services, including Wraparound Family
Team facilitation, group leadership, and coordination of those providing
services to Wraparound families, and oversees the task assignments of
the Parent Partner and Youth Partner. nbsp; Seneca is committed to

community. Responsibilities * Maintain regular contact with the Parent
Partners and Youth Partners on new case specifics or emerging case
issues. * Ensure that services are delivered in a culturally competent
and relevant manner. * Participate in management of the program by
ensuring compliance of other team members. * Assemble the
participantrsquo;s Family Team within three weeks of case assignment by
interviewing the participantrsquo;s family and identifying family
members, natural supports, agency representatives, and other significant
persons. * Coordinate the meeting with the participantrsquo;s Family
Team and developed the individualized Plan of Care based on the Family
Teamrsquo;s strengths and needs.nbsp; * Assist the participant and the
participantrsquo;s Family Team to access strength-based mental health,
social services, education services, and other supports and services as
identified by the participant and the participantrsquo;s Family Team. *
Provide or secure support and crisis/emergency services for the
participant and/or the participantrsquo;s Family Team including services
available through the Provider Network. * Ensure that the participant
and participantrsquo;s Family Team are involved in all phases of
determining goals and needs to be identified in the Plan of Care * Act
as a liaison for the participant and participantrsquo;s Family Team when
new services or resources need to be sought or developed, and modify the
Plan of Care accordingly. * Provide transportation of the participant to
appointments or to access services as required.nbsp; Qualifications *
Bachelorrsquo;s Degree in human services, psychology, child and
adolescent development, social work, or related field from an accredited
college or university required. * Must be at least 21 years of age. * 1
year experience doing case management and working with children and
their families in a variety of mental health settingsnbsp;preferred. *
Ability and willingness to work independently in the office and in the
community. * Clearance of TB test, fingerprinting, and any other
State/Federal requirements. * Own vehicle, valid California Driver's
License and insurance. Schedule * Monday - Friday * Flexible schedule
