    SCHEDULE: 32 HOURS PER WEEK, NO NIGHTS, NO WEEKENDS, AND NO HOLIDAYS.

     

    The Licensed Social Worker utilizes individual and group therapy skills to assist patients and their families in achieving and maintaining a maximum level of physical, social and emotional functioning. Responsibilities include working as part of a multi-disciplinary treatment team to advise family caregivers, and patients, and help plan a smooth transition to the next level of care.

    Principal Responsibilities

     

    The Licensed Social Worker:

     

    1. Interviews patients and family members to obtain information about home, social environment, family relationships, health history, patient’s functioning and treatment goals.
    2. Explains patient’s background, needs, and family dynamics to the team and participates in the planning of a comprehensive post-hospital care program for the patient.
    3. Collaborates with Licensed Clinical Social Worker to determine the most effective social work treatment modality and carries out therapy to facilitate the adjustments to disability and resolve psychosocial problems with patient and/or family.
    4. Collaborates with other health care professionals to develop, manage, and measure the effectiveness of the transition program.
    5. Assists patient and family with community integration and referrals including DORS/DOA/Private Attendant Care Services, Home Health referrals, Continuum referrals, School reintegration, and Outpatient Services. 
    6. Identifies eligibility and assists family in applying for entitlements.  Investigates alternative sources of funding for uncovered services in the community.
    7. Participates in professional organizations and activities; assists in the orientation of other health care professionals in understanding the services and purpose of social work.
    8. Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.

    Required Skills Required Experience

    1. Master’s Degree in Social Work from an accredited university required.
    2. Current Illinois designation as a Licensed Social Worker (LSW) required.
    3. Demonstrated understanding of the principles, practices, techniques and professional standards in the field of social work.
    4. Knowledge of community resources, health care system and interdisciplinary functioning.
    5. Interpersonal communication, advocacy and analytic skills required to work with patients and families, and various medical professionals within and outside of the organization.

     

    Reporting Relationships

     

    Reports directly to the Clinical Manager at designated site of care.

     

     

    Working Conditions

     

    Normal office environment with little or no exposure to dust or extreme temperature.
