    PRIMARY RESPONSIBILITIES:

    • Develop and maintain social service program, drafting supporting policies, procedures, and guidelines for the program
    • Telephonically assess patient's psychosocial status
    • Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems
    • Works closely with all members of the healthcare team in development of the plan of treatment based on psychosocial assessment and accepted social work interventions
    • Provide social work services including, short - term counseling, community resource planning, and crisis intervention
    • Acts as a resource person for other members of the healthcare team
    • Develops and maintains through knowledge of referral resources throughout the service area
    • Report to NAMM Case Management Nurse variances in patient's condition, medications, or treatment
    • Telephonically monitor patient on a regular basis in regards to psychosocial needs, education, and compliance with treatment plan
    • Maintains up to date knowledge of Medicare, Medical, and other third party reimbursement sources
    • Provides positive customer relations in dealing with patients, families, physicians, team members, hospital departments, referral resources, service agencies and others.
    • Refer to health education and other service departments as necessary
    • Complete all required documentation in a timely manner according to department policy
    • Refer patient to outside community resources, classes, education, etc. as appropriate
    • Case closure as appropriate
    • Collect data necessary for NAMM Case Management reports / QI studies
    • Attend NAMM Case Management meeting and trainings as appropriate

    • Graduate of an accredited Master's degree social work program
    • MSW or LCSW
    • 2+ years of clinical experience with broad range of experience in health care delivery setting (hospital, clinic, physician's office)
    • At least one year of social work experience in a healthcare setting
    • Computer knowledge and proficiency
    • Access to reliable means of transportation to travel in designated area

    • Excellent interpersonal and communication skills (both written and oral).
    • Strong critical thinking and decision - making skills
    • Ability to work in a nurse managed, multi - disciplinary team
