As a Bariatric Counselor / LCSW, you need to know how to: · Provide pre-operative psychological assessments for potential bariatric surgery candidates and determine the appropriateness of surgery from a mental health perspective · Communicate appropriateness of surgery to the clinical multi- disciplinary team · Provide patients with pre-surgical mental health treatment plan · Counsel and educate the patient, family and support system on lifestyle changes and goal-setting pre and post-operatively · Lead evening post-operative support groups. Complete all assessments in a timely manner into the patients medical record · Consult with the bariatric surgeon to coordinate care plans for the patient. Provide and coordinate comprehensive mental health care · Collaborate with the healthcare team to meet patient care outcome goals and program improvement. Provide education to staff and community as needed · Attend and participate in department meetings and participate in multidisciplinary groups as needed or requested · Promote mission, vision and values of SCL Health, and abide by service behavior standards · Perform other duties as assigned Your experience.
