  • Responsibilities

    Provides ongoing psychosocial assessment and treatment planning to patients within a variety of specialty areas in the medical setting.  Collaborates with the multidisciplinary team, lending insight and clinical expertise.  Coordinates complex discharge plans, new placements, changes in level of care, changes in facilities, rehab, and hospice placement. Utilizes resources to enhance patient independence and community adjustment.  Responsible for completion and maintenance of patient’s medical records and submission of patient information as required. This is a full-time position, 4 days a week. 

    MAJOR ACCOUNTABILITIES/CRITICAL RESPONSIBILITIES:

     

    1. Maintains a clinical caseload of patients.

    2. Participates in daily rounds.

    3. Completes psychosocial assessments on all patients and develops treatment and discharge plans from all referred appropriate patients.

    4. Provides crisis intervention services to patients, families and staff.

    5. Provides individual, family and group counseling to enhance coping skills.

    6. Demonstrates knowledge in providing comprehensive clinical case management services for complex discharge plans, new placements, changes in level of care, changes in facilities, rehab, and hospice including receipt of entitlements, advocacy, referral and linkage with schools, the legal system, hospital-based and community resources.  Provides patient and family with information concerning available community resources.

    7. Develops and implements treatment strategies to engage, stabilize support and motivate patients.

    8. Demonstrates ability to follow-up, outreach and coordinate service delivery.

    9. Recognizes, evaluates and develops interventions to address signs of abuse, neglect and domestic violence for all ages and takes the appropriate action including reporting incidents to the proper authorities.

    10. Recognizes, evaluates and develops interventions and/or makes referrals for alcohol/substance abuse cases.

    11. Insures timeliness and documentation of all contacts, interventions and outcomes. Completes required forms and assures that confidentially is maintained at all times.

    12. Effectively delegates tasks to the appropriate skill level.

    13. Participates in program development, performance improvement, and special projects and provides consultation to other departments and staff.

    14. Consults with supervisor for problem solving. Participates in discussion during team meetings and meets for supervision as scheduled.

    15. Maintains knowledge of current developments in the field through reading of professional literature and attendance at relevant seminars and conferences.  Shares knowledge and expertise and serves as a resource to team staff members for complex treatment issues.

    16. Completes required continuous training and education, including department specific requirements.

    17. Demonstrates professional work behavior by following Service Standards and Success factors. Complies with departmental organizational policies and procedures and adheres to external agency requirements.

    18. Provides coverage for absent co-worker, when necessary.

    19. Responsible for performing all other duties as assigned.

     

    Required Skills

    1. Work requires the knowledge of theories, principles, and concepts normally acquired through completion of at least a Master's degree in Social Work, Psychology or closely related field plus three to six months of on the job training and orientation. Worker must demonstrate sound clinical judgment. Bilingual (Spanish/English) preferred.

    2. Licensure by the State of CT preferred.

    3. Requires working knowledge of Medicaid and Medicare/health care financing.

    4. Work requires the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems.

    5. Work requires the ability to prepare detailed assessments of patient referrals, conduct individual or family sessions, or perform other duties requiring a comparable level of communication skill.

    6. Work requires ability to communicate orally and in writing.

    7. Work requires checking documents for accuracy.

    Required Experience
